Description
In accordance with FAR 6.302-5, this Notice of Intent is to inform all parties the Department of Veterans Affairs, Network Contracting Office (NCO) 19 located at 6162 South Willow Drive Suite 300, Greenwood Village, CO 80111 intends to award a sole source procurement for the following requirement to large business, Carefusion 2200, Inc., 3750 Torrey View Ct. San Diego, CA 92130: Carefusion is the OEM for BD Pyxis equipment and the sole authorized servicer for all repairs and maintenance services. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT BD Pyxis Service Agreement: Contractors shall provide service repair and preventative maintenance (PM s) for all BD Pyxis equipment located at Salt Lake City VAMC as described herein. Contractor shall provide 24x7x365 Technical Support via telephone and remote access to installed systems. Systems and processes are in place for thorough documentation of issues reported by the customer. Supervisors can listen to and monitor progress on calls with automatic notifications that trigger escalation of issues within specific time parameters. Calls are recorded to monitor customer satisfaction and for later evaluations and reviews of employee performance. Automated systems are in place that recommend required staffing levels throughout the day and can be used to project staffing requirements into the future. A key element of Technical Support is remote access using console sharing software which allows CareFusion to diagnose and solve problems as well as provide user training and other valuable services. The technician can view the same image seen by the local user and both can operate the keyboard and mouse to interact with the screen. Technical Support can then provide instructions to authorized customer representatives regarding actions to be taken to correct or assist in correcting the problem. Additionally, CareFusion equips every system with software to diagnose and troubleshoot problems. Using diagnostic software, CareFusion Technical Support can assess a component failure and quickly arrange for a part replacement. The CareFusion Technical Support toll-free number is the primary point of contact for any maintenance service, repair, or other assistance that is required. On-site Field Support Contractor offers on-site field support for diagnostic and repair services, as well as preventive maintenance. Contractor provides fully qualified technicians to perform necessary maintenance service and repairs to CareFusion equipment to ensure optimum operation and efficiency of the equipment is maintained, and that proper functioning of the equipment conforms to CareFusion's (OEM) standard specifications. Contractor will keep the system in good operating condition, meeting all published equipment requirements. Diagnostic and Repair Services If the equipment needs repair, servicing, or troubleshooting that cannot be performed remotely, contractor dispatches an authorized CareFusion Service Technician. The Technician will arrive during the pharmacy s standard hours of operation. The minimum standard response time will be no later than the following workday after notification of equipment malfunction. Technicians work in and around the pharmacy workflow to be as non-intrusive as possible, and they work efficiently to bring the products back online. Preventive Maintenance Preventive maintenance visits are to be scheduled at the convenience of the Salt Lake City VAMC. The visit includes scheduled maintenance, inspections, replacement of defective and worn parts, and testing services in accordance with CareFusion OEM specifications. Part Replacement All parts, labor, and travel expenses to perform maintenance service and repairs (preventive or corrective) are included in the monthly support fee. Parts requiring replacement to maintain the equipment in accordance with CareFusion s specifications will be replaced on a rebuilt or new part basis. 1 PREVENTIVE MAINTENANCE: See above 2  REPORTING:  On-site repair is supplied by this contract 3   PERFORMANCE:  The Contractor must respond to service calls within four hours of notification and return said call.  4   HOURS OF WORK: This contract include all necessary service calls 24 hours a day 7 days a week. 5    FAILING TO RESPOND:  Failure to perform any of the services as set forth in this contract will be considered for invoking provisions of Default. 6   SAFETY STANDARDS:  The contractor shall perform electrical safety measurements on all equipment under this contract in accordance with manufacturer s specifications.  All equipment will be subject to inspection by VA Engineering Section (Biomedical) personnel to verify compliance with these standards. 7   ADDITIONAL SERVICES:  The Contractor guarantees all equipment covered in this contract shall be in optimum working condition at the contract expiration date, provided the Contractor is notified of deficiencies at least one (1) day before the contract expiration date.  All changes, updates or retrofits made on any component or system shall be annotated on station equipment manuals and records.  Services also include recording all routine work, corrections and repair work in the equipment log. 8   AUTHORIZED SERVICES:  Only those services specified within the contract are authorized.  Before performing any service or repair of a non-contract nature, the Contracting Officer or his/her designee must be advised of the reason for this additional work.  If appropriate, the Contracting Officer or his/her designee may authorize the additional services or repairs under a separate purchase authorization.  Contractor is cautioned that only the Contracting Officer or his/her designee may authorize additional services or repairs and reimbursement will not be made unless this prior authorization is obtained. 9   RECONDITIONING:  Reconditioning and/or extensive repair work determined not to be economically feasible by the Contracting Officer is not included in this contract and the Government reserves the right to obtain such services from other than the listed Contractor. 10  DISCONTINUANCE OF SERVICE:  The Government reserves the right to terminate service on a particular machine meeting replacement criteria upon thirty (30) days written notice to the Contractor with payment to be prorated. 11  SUBCONTRACTING:  Subcontractors are used on a specific list of break fix items deemed low risk by the Contractor (CareFusion). Subcontractors are provided with all procedures and remote support given to the trained CareFusion Technician. 12  PARTS:      (a)  Only new, standard parts shall be furnished by the Contractor.  All parts shall be of current manufacture and have versatility with presently installed equipment.               (b)  All newly installed replacement parts become the property of the Government.  Replaced parts are to be disposed of by the Contractor after obtaining approval from the Contracting Officer's representative. 13  TEST EQUIPMENT: The DVA Medical Center will not furnish parts and/or test equipment for the performance of this contract.  It is the responsibility of the Contractor to bring the appropriate equipment and/or supplies necessary to complete the work as required within. 14  PERFORMANCE CONFERENCE:  If appropriate, the Contracting Officer will schedule a performance conference for contract orientation purposes with the Contractor receiving award. 15  HAZARD COMMUNICATION STATEMENT:       1. As a vendor, supplier, and/or contractor under contract to the Salt Lake City VA Health Care System for the purpose delineated in the subject contract, you should be aware of this facility's hazard communication program.  By the provisions of your contract, you may be required to work in certain areas where there are hazardous chemicals in use.  Examples include, but are not limited to, areas in Pharmacy Service, Engineering Service, Environmental Management Service, Dental Service, Acquisition & Material Management Service, Radiology Service, Laboratory Service, Nutrition and Food Services, and Research Service.       2.  It is the responsibility of each Contractor to familiarize himself with the specific area(s) in which his contract requires him to work.  This shall include the Contractor introducing himself to the exposure zone supervisor.  The Contractor may obtain from the exposure zone supervisor the nature of the on-going Salt Lake City VA Health Care System work and the type of chemicals used.  In addition, the Contractor should also learn the procedures for obtaining, in an emergency involving the Contractor, the material safety data sheets (MSDS) pertaining to hazardous chemicals in the exposure zone.       3.  It is also the responsibility of each Contractor to meet certain criteria.       These include:             a.  Prior to the beginning of any contract, provide the Contracting Officer's Technical Representative (COTR) with a complete list of all chemicals to be used by the Contractor on the facility premises.  Appropriate MSDS shall accompany the list of chemicals.  The COTR shall forward a copy of this list and the MSDS to the Salt Lake City VA Health Care System Industrial Hygienist (138).             b.  The Contractor shall ensure that all chemical containers are properly labeled and MSDS for each chemical shall be available on the Salt Lake City VA Health Care System premises.  MSDS shall be maintained by the senior representative of the Contractor at this facility.             c.  Protective equipment for Contractor personnel shall be the responsibility of the Contractor.             d.  All other DVA and OSHA regulations shall apply as appropriate.       4.  In compliance with paragraphs 2. and 3. above, it shall be the responsibility of the Contractor to convey to his subordinate personnel and sub-contractors the appropriate information regarding hazard communication. SPECIAL CONTRACT REQUIREMENTS 1 INFORMATION SECURITY: The C&A requirements do not apply; a Security Accreditation Package is not required. Incidental exposure to protected health information may occur from accessing the work site. Staff will follow all Privacy & Security policies and procedures. Salt Lake City VA does not allow storage media to leave VA premises. This includes hard drives, thumb drives, CD ROM, floppy drives, and certain memory sticks.  These must be removed and turned into Salt Lake City VA Information Security Officer before equipment is removed. SALIENT CHARACTERISTICS The material/service must be compatible in all aspects (form, fit and function) with existing systems presently installed/performing. The equipment installed by the original manufacturer (OEM) and owned by the VA Salt Lake must have service by the incumbent contractor and materials (OEM) compatible with that dispensing equipment and is performed by authorized service technicians to ensure warranty continuation. Software and operating system are unique to this vendor/manufacturer and as necessary updates and support are essential to proper functionality/ medication dispensing.
