Description
This Sources Sought/Market Survey Notice is being conducted by LTC Charles S. Kettles VA Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105 to identify all sources that are capable of providing Onsite Job Hazard Analysis (JHA) and Relational Database services for LTC Charles S. Kettles for the period of December 15, 2023 December 14, 2024. The Contractor shall be responsible for developing a MS Access relational database capable of developing task-specific JHA procedures. The contractor shall work closely with a representative from the LTC Charles S. Kettles VA Medical Center to determine the template format for the JHAs which will, at a minimum, contain the following information: JHA Header Section · Applicable Service(s) · Affected Job Title(s) · Job Location · Date Performed · Individual Performing Analysis · Primary Hazards · Personal Protective Equipment (PPE) Required JHA Body Section · Basic Job Steps · Existing/Potential Hazards; including assigned Risk Rating · Recommended Actions and Control Measures JHS Sign-off Section · Name of Trainer and Individual Reviewing the JHA · Signature Section · Date In addition, the contractor shall perform On-site Job Hazard Analysis (Field Observations and JHA Development). The contractor shall visit the LTC Charles S. Kettles VA Medical Center to observe workers performing tasks as identified in advance by the LTC Charles S. Kettles VA Medical Center Safety Department. As a part of this process, where determined to be beneficial, the contractor shall take photographs to supplement the final JHA.
