- Charleston CMOP Statement of Work The Charleston Consolidated Mail Outpatient Pharmacy, located @ 4136 Carolina Commerce Parkway, Ladson, SC 29456, is a 98,000 square foot facility which is a provider of outpatient pharmaceutical services for 22 VA Medical Centers and associated community clinics. This CMOP will dispense approximately 160,000 prescriptions daily direct to patient. This CMOP operates 2 shifts, 5 days per week with some over-time, and employs 330 staff members. We have approximately 1.5 million dollars worth of operational supplies at any given time in our building but are lacking storage space to house all these supplies. Our coolers take up a huge footprint in our building and this trailer will be attached to one of our shipping docks to be used for dry storage of them and easy retrieval with a forklift. REQUIREMENTS: Bidder must deliver trailer to the Charleston CMOP at the address below. Trailer must be 53 foot long and 8- 8.5 foot wide. Must contain rear door for forklift entry. Height must equal dock for forklift use Must contain 2 axles. Must be able to hold 40,000 pounds of weight. Trailer must come complete to stand on its own without the semi-truck attached, this will be for dry storage only DELIVERY LOCATION: Dept. of Veterans Affairs Charleston CMOP 4136 Carolina Commerce Parkway Ladson, SC 29456 ****ACCEPTANCE CRITERIA FOR SUBMITTING PROPOSALS (ALSO SEE EVALUATION SECTION BELOW)** 1. All contractors turning in a proposal must include this solicitation document in its entirety. 2. On page 1 box 17a needs to be filled in with your company name, address, phone and SAM Unique Entity ID. 3. On page 1 boxes 30a/b need print signature, signed and date. 4. Section B.2 Price Schedule box below needs to be filled in. 5. Please let me know if you have a trailer readily available AND the timeframe it can be delivered to the Ladson SC, Charleston CMOP. This can be in your email back to me with this solicitation attached. 6. Picture(s) shall be included with your offer of the trailer when you submit your package
