- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. VHA RPO West NCO 17 Health Care Resources Branch I is planning to solicit for a vendor who can provide Community Based Outpatient Clinic (CBOC) services. Services Required: The VA North Texas Health Care System (VANTHCS) requires the following services to be provided in a private hospital, office, or clinic environment to Veterans, primarily residing in Grayson County, Texas, and the surrounding counties catchment area. Primary Care CBOC: Offers both medical and mental health care (physically on site and by telehealth) and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and mental health services. Mental Health services will be provided VA Mental Health staff by a combination of on-site and telehealth methodologies. Place of performance: This facility shall be located within the city limits of Sherman, TX. Sherman, TX is located approximately 75 miles from VANTHCS. Based on this growth rate, it is projected that enrollment at the Sherman CBOC will increase from approximately 3,700 enrollees to 4,500 enrollees over the ten-year term of the contract. VHA RPO West NCO 17 Health Care Resources Branch I is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Type of Contract: If this sources sought notice leads to a future solicitation, the Government intends to award a fixed price indefinite delivery indefinite quantity (IDIQ) contract. The performance period shall be one (1) base year plus four one-year options. The proposed NAICS Code is 621498, All Other Outpatient Care Centers. Interested parties shall provide a brief overview of their firm's capabilities and experience with projects similar in scope to this project. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization SAM Unique Entity ID Number d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size under NAICS 621498 f. Socio-economic status (IF SDVOSB or VOSB - provide SDVOSB or VOSB verification) g. Whether your company has experience setting up and operating a VA CBOC. h. Whether you have experience with the current Patient Aligned Care Team (PACT) modeled clinics. i. Whether your company has experience using the capitated (per member per month) rate method. Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 621498. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than October 13, 2023, by 5:00 PM ET.
