All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Louis A. Johnson VAMC 1 Medical Center Dr. Clarksburg WV 26301 General Intention: It is the general intention of the Louis A. Johnson VAMC located at 1 Medical Center Dr. Clarksburg WV 26301 to acquire contractor services for the purposes of preventive maintenance for laundry plant equipment. Contractor Requirements: Following the establishment and verification of all equipment meeting manufacturer specifications the maintenance contractor will maintain all equipment at the required specification throughout the life of the contract. Spare parts, maintenance material: The contractor shall provide all consumable parts, or maintenance materials required for the continued service life of the equipment. Consumable parts are defined as items requiring replacement at recommended pre-determined intervals by the equipment manufacturer. The contractor shall provide a monthly written status report to the VAMC COR. Reports shall include details of but not limited to; PM s completed, consumable parts used, upcoming PM activities requiring equipment disruption. Vendors must submit a checklist detailing items that will be addressed and covered in the contract for each piece of laundry equipment during each site visit/ preventive maintenance service. The checklist must include all parts and labor that will be covered in the contract. Checklists must be submitted along with the vendors bid. These checklists should mirror the preventive maintenance schedules in the maintenance manuals for each piece of laundry equipment. These checklists must be used during the site visits/ preventive service maintenance service and signed by the technician performing the work indicating that all applicable services were completed for each piece of laundry equipment. All service adjustments will be made in accordance with manufacturers specifications using required tools and procedures. Vendor shall be responsible for cleaning of the work and equipment upon completion of work. All accumulated waste shall be removed daily. All oil, lubricant, and any other material substance creating an unsafe operating area shall be removed daily. The vendor shall be responsible to dispose of all hazardous materials in accordance with state and federal regulations. All used hazardous products shall be removed daily. Contractor shall perform preventive maintenance activities on a recurring schedule at a minimum of six-week frequency. The contractor shall submit a schedule of PM site visits at the beginning of the contract year. Increased frequency of preventive maintenance may be required at no additional cost to the government based upon manufacturers requirements. The government reserves the right to dispatch the vendor for follow on site visits at no additional cost due to non-compliance with PM requirements, or government non-acceptance of quality of work performed.
