- Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04, effective 06/02/2023. This solicitation is set-aside for: 100% Small Business (SB) The associated North American Industrial Classification System (NAICS) code for this procurement is 811210 with a small business size standard of $34.0 Million. The FSC/PSC is J074. The Department of Veterans Affairs, Network Contracting Office 22 is seeking a contractor to perform services to dismantle, transport, and crate a Lektriever Series 2000 electronic filing cabinet at the Greater Los Angeles VA Healthcare System, West Los Angeles VA Medical Center; located at 11301 Wilshire Blvd. Los Angeles, CA 90073-1003. All interested companies shall provide quotations for the following: 1. Scope of Work: The contractor shall provide required supplies, equipment, and personnel needed to facilitate the removal and disposal of a Lektriever Series 2000 electronic filing cabinet. Contractor shall provide equipment, supplies, and perform work in strict accordance with the Statement of Work, schedule, and all other supporting documents required for this contract. 2. Tasks: Dismantle 1 ea. Lektiever Series 2000 in Building 218, Room 228. Transport Lektriever 2000 parts from Building 218 to Warehouse Bldg. 297 using Vendor-provided truck, dolly, other material handling equipment as needed for job. Provide all material for construction of suitable crating for Lektriever and associated trays to be constructed in warehouse Bldg. 297. Provide labor for building crating for Lektriever and associated trays for warehouse storage and subsequent transport of 1 Ea dismantled Lektriever 2000 with trays. All quotes shall be sent to the Network Contracting Office (NCO) 22 Contract Specialist, Darryl Handberg at Darryl.handberg@va.gov. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. This purchase order will consist of a base period of 30 days (09/27/2023 10/27/2023) pricing should reflect this. The following are the decision factors: Price and Past Performance. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any.
