- Instructions: This Sources Sought is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this notice is marked as proprietary and will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. This action is to perform market research to determine if there is a sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small or emerging small business firms; or (4) large business firms to set aside a multiple award contract. SCOPE OF WORK: The intent of this Contract is to have one five-year IDIQ contract for wheelchair van services to transport patients including physically or mentally handicapped individuals in wheelchairs and aged and infirm persons, their attendants, and personal belongings throughout the VA Butler Healthcare service area. The Contractor agrees to furnish non-emergent, wheelchair van services, 24-hours per day, seven days per week, (to include all federal holidays) for all patients of the VA Butler Healthcare located at 353 North Duffy Road, Butler, PA 16001; Community Living Center (CLC) 325 New Castle Road, PA 16001; and their Community Based Outpatient Clinics (CBOCs) in Lawrence, Mercer, Clarion, and Armstrong Counties, to include the Cranberry TWP CBOC in Butler County. Patients may also be transported to the VA Pittsburgh Health Service (VAPHS) in Oakland or Aspinwall, Pittsburgh, PA. Care provided during transfer of a patient shall be according to the orders of the attending physician. Attached is a DRAFT copy of the PWS. Please review for additional information. The anticipated contract issued will be one five-year IDIQ. All interested firms who can meet the requirement should respond, in writing, and provide the following: 1. Please describe your business in a Capabilities Statement. 2. Please provide your SAM UEI number. 3. Please provide your location / address. 4. What is your Socio-Economic status? 5. Are you capable of picking up and dropping off at all required locations? 6. The assigned NAICS code is 485991. Please update me if you feel there is a better NAICS for this requirement.
