- 36C25023Q1091 SPECIAL NOTICE NOTICE OF INTENT TO SOLE SOURCE Verifit 2 Audioscan The Cleveland Department of Veteran Affairs Medical Center, 10701 East Blvd Cleveland, OH 44106 intends to solicit and negotiate with only one source under the authority of FAR 13.1061(b)(1)-Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available. The NAICS code is 334510 with a small business size standard of 1,250. The VA intends to solicit and negotiate with e3 Diagonstics 3333 N Kennicott Ave. Arlington Heights IL 60004. The Audiology department is seeking the purchase/replacement of our Audioscan Verifit 2 equipment that is required for audiology testing of patients. The following equipment is being requested: 8521199 ETY-VA-201-ADDTL Product: NOAH Module Audioscan additional NOAH Module 8521186 ETY-VA-113 Product: VF2 Floor Speaker Kit 8513219 ITA-8513219 Product: Lyra Interacoustics Lyra DPOAE 8506058/ITA-8506058 Product: Interacoustics Laptop Computer - For use with all Eclipse models Operating System: Windows 10 Pro Printer not included. 8517470 GSI-8013065 Product: OtoAccess Database Training & Installation Other persons may identify their interest and capability to respond to this requirement by Monday August 28, 2023 at 12PM EST. Such capabilities information will be used solely for the purpose of determining whether to conduct this procurement on a competitive basis. All information shall be furnished at no cost or obligation to the Government. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
