- Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC Number 2023-24, Effective Date 06/02/2023. This solicitation is set-aside for: SDVOSB (service-disabled veteran-owned small business concern). The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 750 Employees. The FSC/PSC is 6515. The Surgical and SPS Services at the Northport VAMC is seeking to purchase a Depuy/Mitek Surgical System shoulder tray. All interested companies shall provide quotations for the following brand name or equal requirement: Line Item Description Quantity Unit of Measure 214140 EXPRESSEW III suture passer without hook 1 EA 214641 clever hook, left 1 EA 214640 clever hook, right 1 EA 214623 tissue liberator, blade up 1 EA 214624 tissue liberator, blade down 1 EA 214600 penetrating grasper, straight 1 EA 214601 penetrating grasper pen, 35deg up 2 EA 214626 suture manipulator 1 EA 214617 calibrated probe 1 EA Background: Orthopedic surgeons at Northport VAMC require specialized instrumentation as well as implantable hardware (screws, plates, rods, etc.) to treat fractures of the small bones in the shoulder. Currently, these specialized shoulder trays (inclusive of instrumentation and hardware) are rented on a per case basis from outside vendors. This introduces a variety of issues including late or missed deliveries, incomplete trays missing crucial items, delays in being cleared by SPS, etc. These issues routinely result in delays and cancellations of procedures (sometimes when the veteran has already been sedated for the procedure). Scope: The surgical and SPS services require a shoulder tray kit with specialized instrumentation and hardware to treat rotator cuff, shoulder replacement surgeries (and other various shoulder surgeries) as well as a replenishment stock of hardware to minimize downtime in between procedures. As stated above, the current system has created several patient care issues such as delayed and cancelled procedures including the need to refer patients to outside facilities for care. Owning and maintaining our own shoulder tray system will also eliminate the need for further cost involved in renting the trays from outside vendors. Additionally, it will allow for cases to be booked on an emergency basis without the 48 hour or greater lead time that is involved in the rental process and internal sterile protocol thus minimizing pain, discomfort, and risk of complications for the veteran. The Depuy/Mitek surgical system is the required shoulder tray system of the orthopedic surgeons at Northport VAMC. It provides a complete set of crucial instrumentation that will meet the needs of the surgeon for the various types of shoulder procedures performed. The replenishment trays will allow for minimal downtime as the trays and instrumentation can be sterilized in house and implanted items can be replenished from our own stock, then replaced as needed by the prosthetics service in between procedures. The Depuy/Mitek system is a complete kit that provides all needed equipment for each, it was also found to be the most economical and readily available for replenishment by the service, they will however accept Brand Name or equal based on the salient characteristics and specifications highlighted in this statement of work and noted in attached product information. The purchase order period of performance is 45 days after receipt of award. The following are the decision factors: Award shall be made to the quoter, whose quotation, offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
