- SOURCES SOUGHT NOTICE This is a SOURCES SOUGHT NOTICE (SSN) ONLY. The U.S. Government is conducting market research only to determine the availability of qualified sources capable of providing GeneXpert System Maintenance Service for the Roseburg VA Healthcare System V. Potential Contractors are invited to provide a response via e-mail to Contract Specialist at sangmi.kim@va.gov by August 09, 2023 12:00 PM (PT). Responses will be used to determine the appropriate strategy for a potential acquisition. Please clearly identify any information your company considers sensitive or proprietary. This notice is issued solely for information and planning purposes - it does not constitute a Request for Quotation (RFQ), or a promise to issue an RFQ in the future. This notice does not commit the U.S. Government to contract for any supply or service. Further, the U.S. Government is not seeking quotes, or proposals at this time and will not accept unsolicited quotes in response to this sources sought synopsis. The U.S. Government will not pay for any information or administrative costs incurred in response to this notice. Submittals will not be returned to the responder. Not responding to this notice does not preclude participation in any future RFQ, if any is issued. BACKGROUND: The Roseburg VA Healthcare System Pathology and Laboratory Medical Services located at the Rose VA Medical Center, Roseburg, OR has a requirement for a Contractor to provide the Preventative Maintenance Service for GeneXpert System including Parts, Labor and Travel. Please see the draft Statement of Work below for requirements. Please state if your FSS Schedule is under Group 66 III Cat 605-2A________ 2) Please submit a brief capability statement (maximum two pages) with enough information to demonstrate to the Veterans Affairs that you have resources, trained and authorized personnel to provide the Preventative Maintenance Service for GeneXpert System as required in the attached Draft. **Draft Statement of Work** As of 07/31/2023 Contract Title: New Base + 4 service contract for GeneXpert(Serial Number: 815130) at the Roseburg VA Healthcare System Laboratory. 2. Background: Individual piece of equipment/instrument purchased needs service contract for preventative maintenance to remain compliant and useable. 3. Scope: The work will be performed for the Roseburg VA Healthcare System Pathology and Laboratory Medical Services. Equipment/Instruments include: Model: GENEXPERT XVI Serial Number: 815130 ANALYZERS: LAB: MOLECULAR ASSAY: INFECT ORGANISM 4. Specific Tasks: ADVANTAGE LEVEL SERVICE CONTRACT INCLUDES: 24x7 Phone Support Onsite 5x10 hours (8AM 6PM) Annual Preventative Maintenance Check-up performed by Field Service Engineer Includes Parts, Labor & Travel Software Upgrades included 98% Uptime Guarantee Advantage Service Agreement for GeneXpert s/n: 815130 Under this Contract, Tech Support, maintenance and repair for covered GeneXpert systems shall be prioritized over customers without a service plan. As part of the services offered under this Contract, The Contractor shall provide, at no additional charge, all parts, labor, and return shipping costs associated with GeneXpert/GeneXpert Infinity system repair incurred during the duration of this Contract. Routine parts replacement includes any device associated with the system, except for parts provided in calibration kits or consumable items such as printer cartridges or other supplies identified in the Operator s Manual. All Plans above include telephone technical support according to Section 8 below. Service visits exclude holidays recognized by the Contractor. During this Contract, Customer is entitled to any and all GeneXpert/GeneXpert Infinity system updates (bug fixes etc.) and software upgrades (software feature enhancements) at no additional charge. This Contract does not cover any upgrades to hardware or third party software. 5. Performance Monitoring (if applicable): POC laboratory microbiology lead to submit preventative maintenance paperwork provided by the contractor to Biomed for annual PM sticker update. 6. Security Requirements: The Contractor required to schedule appointment with POC before arriving on campus for preventative maintenance check and service of instrument. The Contractor required to check in at Roseburg VA campus police station before entering laboratory. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI): Equipment/Instruments included: Model: GENEXPERT XVI Serial Number: 815130 ANALYZERS: LAB: MOLECULAR ASSAY: INFECT ORGANISM 8. Other Pertinent Information or Special Considerations: None a. Identification of Possible Follow-on Work: Preventative Maintenance (PM) stickers updated annually on equipment/instruments listed above. b. Identification of Potential Conflicts of Interest (COI): None c. Inspection and Acceptance Criteria: Equipment/instruments working at 100%. 9. Risk Control. Service contract will keep the equipment/instruments compliant with regulatory preventative maintenance and useable to serve our veterans. 10. Place of Performance. The work will be performed for the Roseburg VA Healthcare System Pathology and Laboratory Medical Services. 11. Period of Performance. New base +4 (total 5 years) Base Year 10/01/2023 - 9/30/2024 Option Year 1 - 10/01/24 - 9/30/25 Option Year 2 - 10/01/25 - 9/30/26 Option Year 3 - 10/01/26 - 9/30/27 Option Year 4 - 10/01/27 - 9/30/28 12. Delivery Schedule. On Site between 8:00AM 6:00PM local time in the Continental U.S., 5 Days a week. The Contractor shall provide telephone technical support through its Technical Support Call Center.
