- This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation PIID 47PM1023R0025 is a Request for Proposals for Medical Surveillance for White House Service Center located in Washington, DC 20506. Due to the nature of this procurement, contractors are required to be within a thirty (30) mile radius of 725 17th St NW Washington, DC 20506-0200. The period of performance is projected to be 365 days from the award date. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-04. This procurement is a Total Small Business Set-Aside. The NAICS Code has been identified as 621111 Offices of Physicians with the small business size limit of Annual Revenue of $12 million.� The General Services Administration is procuring Occupational Medical Surveillance Program (OMSP) services for forty-four (44) White House Service Center Federal employees. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform OMSP services as described in the requirements of the attached statement of work and in accordance with applicable regulations. Individual line items, quantities and units of measures will be included in the solicitation form SF33, please reference the SF33 for full information on the breakdown of line items. If identified line items need to be further broken down, please provide proposed breakdowns on the Contractor Pricing Breakdown sheet. Solicitation Provisions and Contract Clauses will be attached and should be reviewed and responded to as required.� The solicitation is due no later than Tuesday 8/15/2023 at 4:30 PM EST via email to the contracting officer, Tatiana Oliver, tatiana.oliver@gsa.gov and contract specialist, Andrew McElroy, andrew.mcelroy@gsa.gov. For your proposal to be considered complete and responsive, please ensure that the following documents are submitted no later than the due date listed above.� Evaluation factor for the basis of the award� is the lowest price technically acceptable. While the SF33 provides an estimated overall cost, due to the individualized nature of this procurement the contractor�s proposal shall include line items for each service proposed, the proposed line items will determine the lowest price technically acceptable. Required Documentation for Responsive Quote Standard Form 33 Please review the entire form for accuracy, then fill out boxes 12-18 as applicable. Boxes 16-18 are required for a complete and responsive proposal. Contractor Proposal Please provide a proposal no more than 20 pages in length with recommended examinations, risk assessments, and labs based upon the statement of work and OSHA requirements for the job series as identified in the statement of work. For each service and job series, please mark as either required, optional, and/or if medically indicated. Each service should also include a per unit price point. Due to the nature of the individualized requirements, please provide your best total estimate for all employees identified in the statement of work. However, the primary pricing consideration for evaluation will be the individual line item pricing.� Contractor Pricing Breakdown The contractor pricing breakdown should be divided by indicated job series and potential examinations that align with your proposal. Please enter unit prices and identify whether the item will be required, optional and/or if medically indicated. Representations and Certifications Fill in your company�s information at the top. Please review the clauses and ensure that applicable selections are made throughout the document.
