- 631-21-010 Repair Foundation Walls and Building Drainage Systems Questions and Answers Question 1: As per page 8 of the RFP, please provide the ""attached"" CALCULATION OF SELF-PERFORMED WORK. Answer 1: See attached. Question 2: Per page 14 of the RFP, please provide the required template for the itemized price breakdown. Answer 2: No required template. Should provide sufficient detail to permit a complete analysis of labor burden, materials, equipment, transportation, supervision, disposal costs and overhead and profit and shall cover all work involved in the contract. Question 3: As Per Evaluation Factor 2. Management Approach - Technical Qualifications, Section D. Effective Construction Safety Plan and demonstrated satisfactory record of safety performance on past projects on page 10 of the RFP, please provide the ""attached"" Pre-Award Contractor Evaluation Form for Safety. Question 4: Please confirm whether the VA will accept a self-certification for the below required information listed on page 10 of the RFP: ""certification that the bidder/offeror has no more than three (3) serious or one (1) willful OSHA or any EPA violation(s) in the past 3 years."" If self-certification is not acceptable, please confirm who should issue said certification. Answer 3 and 4: Section D has been revised to read: Effective Construction Safety Plan and demonstrated satisfactory record of safety performance on past projects Using the information submitted in response to the solicitation, the Government will evaluate the offeror s ability to comply with an effective construction safety plan submitted in response to the requirements of the solicitation. The offeror shall develop a proposed plan in response to this evaluation sub-factor. Additionally, the proposal shall include documentation regarding the successful training of personnel in accordance with OSHA requirements (i.e., 30-hour or 10-hour completed OSHA training.) The narrative demonstrates familiarity with and/or plans to adhere to OSHA requirements and other safety requirements incorporated in the solicitation. To be evaluated as acceptable the offeror must submit a proposed plan in response to this evaluation factor addressing the requirements of the safety and training requirements. The offeror shall submit a narrative response for this evaluation factor. Removed language: Additionally, the proposal shall contain relevant information regarding any safety accidents or violations and corrective action taken or demonstrates no documented violations. Offeror shall demonstrate that the company has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past three years and has an Experience Modification Rate (EMR) of equal to or less than 1.0. Simply having an EMR greater than 1.0 does not eliminate a contractor from selection. The Offeror will be allowed to submit an offer with additional information explaining the EMR; and the CO will need to make a responsibility determination based upon all the usual factors. Further, if the offeror is deemed non-responsible, but otherwise eligible for award; the matter will be referred to the SBA for a Certificate of Competency. Offeror shall complete the attached Pre-Award Contractor Evaluation Form for Safety and submit it with your narrative response to this sub-factor. To be evaluated as acceptable the offeror must submit a proposed plan in response to this evaluation factor addressing the requirements of the safety and training requirements. The offeror shall also complete the attached Pre-Award Contractor Evaluation Form for Safety and submit it with your narrative response for this evaluation factor. NOTICE-SAFETY COMPLIANCE REQUIREMENTS Safety or Environmental Violations and Experience Modification Rate All Bidders/Offerors shall submit the following information pertaining to their past Safety and Environmental record: (The information as outlined below shall be submitted at the time of proposal submission. 1. VIOLATION INFORMATION: The information shall contain, at a minimum, a certification that the bidder/offeror has no more than three (3) serious, or one (1) willful OSHA or any EPA violation(s) in the past three years. 2. EXPERIENCE MODIFICATION RATE: All Offerors shall submit information regarding their current Experience Modification Rate (EMR) equal to or less than 1.0. This information shall be obtained from the offeror s insurance company and be furnished on the insurance carrier s letterhead. Self-insured contractors or other contractors that cannot provide their EMR rating on insurance letterhead must obtain a rating from the National Council on Compensation Insurance, Inc. (NCCI) by completing /submitting form ERM-6 and providing the rating on letterhead from NCCI. Note: Self-insured contractors or other contractors that cannot provide EMR rating on insurance letterhead from the states or territories of CA, DE, MI, NJ, ND, OH, PA, WA, WY, and PR shall obtain their EMR rating from their state-run worker s compensation insurance rating bureau. 3. Simply having an EMR greater than 1.0 does not eliminate a contractor from selection. The Offeror will be allowed to submit an offer with additional information explaining the EMR; and the CO will need to make a responsibility determination based upon all the usual factors. Further, if the offeror is deemed non-responsible, but otherwise eligible for award; the matter will be referred to the SBA for a Certificate of Competency. Question 5: Does the contractor need to explore and drill? Answer 5: Exploring and drilling is an acceptable method, upon approval and coordination with the COR, for verifying the existing conditions as required. Refer to General Note 1.1 on Drawing No. GI100 and Existing Conditions Note 4 on Drawing No. GI101. Question 6: Can the contractor perform the same type of work in different buildings? Or do we need to start and finish the scope of work for each building? Answer 6: The contractor shall coordinate schedules and phasing with the COR. Refer to Specifications section 01 00 00.1.6.K.1. Per section 01 00 00.1.6.K.2.a, Each Building shall be considered a phase. VA is open to allowing multiple buildings/phases to be done concurrently but must be approved by the COR after NTP.
