- STATEMENT OF WORK Seclusion Room Repair for 5th Floor East Department of Veterans Affairs Veterans Health Administration Jack C Montgomery VA Medical Center 1011 Honor Heights Drive Muskogee, Ok 74401 Background: The Jack C Montgomery VA Medical Center (VAMC) in Muskogee, Oklahoma has a mental health ward. This ward has padded seclusion rooms to isolate patients in need of therapeutic containment. Two of these rooms need repair,and padding installation. Objectives: The VAMC is requesting a contractor to makes repairs as desctibed below in the scope section for Room 144, and install new padding as described below for Room 146 (i.e. these rooms are located on the fifth floor of building 53). Scope: Tasks Room 144 Fill three (3) 3 x 7 recesses in the existing wall padding in room 144. The potential offerors/contractors shall only utilize and quote Brand Name Only Gold Medal Safety padding. Prepare and re-coat the safety padding in room 144 and fill in any graffiti, wear, or scrapes in the existing safety padding. Repairs shall match the like existing padding and shall maintain integrity and functionality for patient safety. Tasks Room 146 Remove baseboard in room 146. Install Gold Medal Safety padding on the floor of patient room number 146. The floor safety padding is to surround a secured seclusion bed. The potential offerors/contractors shall only quote Brand Name Only Gold Medal Safety padding for this room. The approximate size of the floor is (130 ) 10 -10 x (138 ) 11 -5 = 123 square feet. Reinstall baseboard in room 146, materials to be provided by the installation. Delivery: The contractor shall repair, furnish, install, and/or provide all materials in accordance with this statement of work (SOW). Any changes to the scope of work or scope can only be completed by the contracting officer. Therefore, it is the Contractors responsibility to communicate with the COR who will then notify contracting appropriately. Deliverables Contractor shall deliver all materials needed. Contractor shall provide all labor needed. Delivery location All deliveries shall be scheduled with the VAMC COR. The VAMC has a loading dock that may be utilized by the contractor. No deliverables shall be accepted by VAMC personnel without the presence of the contractor. All deliverables provided by the contractor can be stored in room 144 and 146, on the 5th floor, while each room is under construction. The VAMC will not authorize any storage outside of the construction areas. The construction area is a secure and sensitive area. Thus, the contractor will ensure all tools, materials, or any other foreign materials are always secured. Supporting documentation The contractor shall provide all safety data sheets for all materials provided, used, and stored for this project. The contractor shall ensure to follow and abide by the SOW, and ensure that all work completed is in compliance with industry standards. Additional information The contractor shall ensure that all materials, foreign to the sensitive area, are always secured. This is to ensure all patients do not have access to any foreign materials. Government-Furnished Property, Material, Equipment, or Information (GFP, GFM, GFE, GFI) The VAMC shall not provide any material, labor, or storage not explicitly written in the SOW. The VAMC shall provide access to the secured floor, for the duration of the project, upon schedule approval from the COR. The VAMC shall provide any codes, locks, or other security mechanisms to ensure no foreign materials are left available to any patients on this floor. Security: The construction area for this project is highly sensitive and heavily monitored. Due to the highly sensitive nature of this floor, all access shall be coordinated through the COR. The contractor shall never assume any right to privacy for the duration of construction period. This floor of the hospital is always secured and monitored by VAMC nursing, security, and regulatory personnel. The contractor shall adhere to all commands issued to them by the charge nurse, Police Officers, CORs, or any other VAMC personnel for the duration of time on this floor. Commands may be given due to security and safety concerns. Time to convey the nature of any issuances may not be available. Once access to the construction area is granted, for any workday, the contractor shall ensure all tools, materials, and personnel are secured and not accessible to patients. Applicable Documents: All the documents listed below shall be abided by and followed for the duration of this project. Place of Performance: All work will be located at: 1011 Honor Heights Dr. Muskogee, Ok 74401 Specific location is in building 53, 5th floor, East wing, room 144 and 146. Period of Performance: The contract shall be completed within forty-five (45) days from notice to proceed. Hours of Operation: Contractor Shall complete work during normal business hours 9:00am to 4:00 PM Central Time. If after hours work is required the contractror shall contact the COR and receive the COR s approval before conducting such work. Holidays: Under current definitions, five holidays are set by date: New Year s Day January 1 Juneteenth National Independence Day June 19 Independence Day July 4 Veteran s Day November 11 Christmas Day December 25 If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday. The other six are set by a day of the week and month: Martin Luther King Day Third Monday in January Washington s Birthday Third Monday in February Memorial Day Last Monday in May Labor Day First Monday in September Columbus Day Second Monday in October Thanksgiving Fourth Thursday in November Changes to the SOW: The Contracting Officer, from information furnished by the COR and Contractor, may direct changes to the drawings and/or specifications that are within the general scope of the contract. If such change is necessary, it shall be accomplished via a written contract modification between VA and the Contractor. Superintendent: The contractor shall ensure to have a superintendent or project manager onsite that is OSHA-30 certified. Meetings: The contractor is required to attend the pre-construction meeting after award, this meeting will be held at a date and time specified by the Contracting Officer to review project scope, site, and requirements, as well as provide answers to any questions.
