Description
THIS IS A Sources Sought Notice ONLY. A Sources Sought notice is a market research tool being used to determine availability and adequacy of potential business sources prior to determining the method of acquisition. �The Medical Readiness Contracting Office-W, Fort Carson, Colorado, has an upcoming requirement to solicit and award a contract for a Radiation (Physics) Survey on radiology imaging equipment. Seeking only Medical Physics Survey on radiology imaging machines: �� All equipment will be tested in accordance with AAPM standards as outlined at �������http://www.aapm.org/pubs/reports/rpt_74.PDF and FDA Title 21 (21CFR1020.30). �Surveyor requirements: 1. Surveyor must be a fully Qualified Medical Physicist, i.e., board certified in diagnostic radiology or board-certified health physicist, certified by the American Board of Radiology or the American Board of Medical Physics. 2. �Surveyor shall be required to provide all test equipment and phantoms necessary for testing. 3. �Hospital will provide an equipment operator to assist in operating the equipment being tested. 4. Survey Documentation required: Surveyor will provide a detailed written report of the survey to include the following: a. site visited b. modality tested c. date of test d. test performed e. results of test f. evaluation of findings 5. �All reports will follow the format list at http://www.aapm.org/pubs/reports/rpt_74.PDF Contractor will label all equipment with: a. Date of Test b. Results of Test c. Date of Retest (if applicable) d. Initials 6. Specialized equipment and reports. a) Surveyor will be required to provide all test equipment and phantoms needed to complete the survey. b) Surveyor will provide documentation of calibration certification of test equipment used. c) Surveyor will provide a detailed report of work done, findings, and evaluations. This notice does not constitute a Request for Quote (RFQ)/Invitation for Bid (IFB)/Request for Proposal (RFP) or a promise to issue an RFQ, IFB or RFP in the future. This notice does not commit the U.S. Government to contract for any supply or service. Further, the U.S. Government is not seeking quotes, bids or proposals at this time and will not accept unsolicited proposals in response to this source sought notice. The U.S. Government will not pay for any information or administrative costs incurred in response to this notice. Submittals will not be returned to the responder. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. However, if a competitive solicitation is released, it will be synopsized on the Government wide Point of Entry, if applicable. It is the responsibility of potential offerors to monitor the Government wide Point of Entry for additional information pertaining to this requirement.
