Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is a combined synopsis/solicitation. Therefore it constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Kayenta Health Center, IT Department is requesting for a Contractor to provide the following COMPUTER CARTS. Per FAR 6.302-1(c)(2) KHC has identified a specific brand name, but will accept brand-name or equal descriptions.� The Contractor shall provide the following items: (20 EACH) C52-2211-1 CareFit Pro Electric Lift Cart, Life Powered, 1 Drawer (20 EACH) SRVC-CRP3YR Three-Year Extended Warranty: CareFit Pro Carts This combined synopsis/solicitation will be in accordance to request for quotation (RFQ) utilizing FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This solicitation is issued as a SMALL BUSINESS SET-ASIDE. SHIPPING ADDRESS: KAYENTA SERVICE UNIT Highway 160, SOUTH OF MILE POST 394.3 KAYENTA, AZ 86033 EVALUATION CRITERIA: This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. The Government reserves the right to issue a firm-fixed price single award or multiple awards to the contractor(s) whose offer represents the Lowest Price Technically Acceptable (LPTA); award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.
