Description
This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Service, Kayenta Health Center is requesting for offers for a contractor to provide a Bulk Funded Purchase Order to receive miscellaneous food and disposable plastic dinnerware on an �As Needed Basis�. This request will be awarded to (2) contractors. Services shall be performed in accordance with the attached Statement of Work (SOW). SERVICES NEEDED: KHC is requesting for miscellaneous food and disposable plastic dinnerware on an �As Needed Basis�. Contractor should be able to provide items in the inventory list attached. These services shall be in accordance with the attached statement of work. PERIOD OF PERFORMANCE:��06/01/2023 - 04/30/2024 LOCATION:� Kayenta Health Center, HWY 160, South Milepost 394.3, Kayenta, AZ 86033. EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered:�Past Performance, Experience, Qualifications, and Technical Capabilities�of the provider. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. SUBMITTAL PROCESS: Email offer directly to the Contracting Officer for this action is: Amber Whitehair, Contract Specialist, 928-697-5074, Amber.Whitehair@ihs.gov.�Limit offer size to 6MB.� Questions may be submitted in writing via email 5 days prior to RFQ Closing Date.� To be considered for award, your offer shall include the following documents or it will be non-responsive and removed from award consideration: Experience: Provide a list of at least (3) past projects of similar scope; include detailed project description, location, dollar amount, and period of performance. Past Performance: Provide a letter of reference from a past customer. Qualifications: Provide evidence that you have the qualified staff to do the work. Technical Capabilities: Provide a detailed description of your service and explain how it would meet the requirements stated in the Statement of Work Supplies: Please provide a listing of the required items needed that your company can provide. The FAR provision 52.212-1 Instructions to Offerors Commercial Items; and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference. CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.
