HCHV Residential Services for Homeless Veterans, San Antonio, TX Please see below This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources, small and other than small businesses, to perform a potential requirement. Respondents are required to be registered and in good standing in The System for Award Management (SAM) https://sam.gov/content/home Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 624221 and the size standard is $9.M. NOTE: Respondent claiming SDVOSB and VOSB status must be certified in US Small Business Administration s VetCert System, https://veterans.certify.sba.gov Project Requirements: NCO 17 on behalf of the South Texas Veterans Healthcare System (STVHCS), Healthcare for Homeless Veterans (HCHV) Program Office located at 7400 Merton Minter, San Antonio, TX 78229 is conducting market research to find available contractors within the greater San Antonio, TX area to provide up to 41 homeless Veterans transitional housing daily. The contractor will provide a therapeutic living environment to homeless veterans. The contractor will provide this care in concert with the STVHCS HCHV Program Office outreach counselor. The veteran will be provided a care plan geared towards permanent housing, sober living, mental stabilization, and permanent employment. Place of Performance: Services shall be provided off site at the Contractor s location. Anticipated period of performance: Base plus four (4) one-year options beginning October 2023. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide the services required in the Statement of work. The firm must include all certifications, qualifications, and training certificates to perform work on the outlined in the Performance Work Statement. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). Synopsis SERVICES REQUIRED. The South Texas Veterans Health Care System (STVHCS) through its Health Care for Homeless Veterans (HCHV) program, is seeking 24-hour-per-day, 7-day-per-week (24/7) community based, recovery model, supportive housing within the city of San Antonio, that provides therapeutic and rehabilitative services in a safe environment to homeless, eligible Veterans. Contractor shall provide housing, case management services, and rehabilitative services within the city of San Antonio for Veterans experiencing homelessness and/or chronic homelessness as defined by the U.S. Department of Housing and Urban Development (HUD), including Veterans diagnosed with severe mental illness and substance use disorders, in community-based residential facilities. PLACE OF PERFORMANCE. Services will be provided in Contractor-provided facility. Facility must be located in San Antonio, Texas and must be appropriate for providing a safe, residential setting while addressing Veterans complex needs through on-site case management and therapeutic support services. This housing must meet the criteria established by both HCHV clinical staff and VA fire and safety officials and defined in the Statement of Work. Contractor must provide Contracted Emergency Residential Services (CERS) to Veterans. Room and Board shall be accessible to the Veteran seven days per week and twenty-four hours per day. Accommodations will include a private or semi-private room, with no more than four (4) Veterans per room, and with a bed and other furnishings such as a dresser, storage, and personal care items. Residents must be assisted in maintaining an acceptable level of personal hygiene and grooming. The environment must be conducive to social interaction, and supportive of the recovery model and of the fullest development of the Veteran s rehabilitative potential. License and Certification. Contract service providers providing counseling services shall be licensed in accordance with all Federal, State, and/or local requirements and certification(s) applicable to the performance of this type of services. This license/certification must be maintained throughout the course of the contract. Contractor shall provide official verification of credentials from the educational institution awarding the professional degree for each of its contract service providers upon request of the Contracting Officer or COR. BASIC SERVICES. Contractor must provide Contracted Emergency Residential Services (CERS) to Veterans. Room and Board shall be accessible to the Veteran seven days per week and twenty-four hours per day. Accommodations will include a private or semi-private room, with no more than four (4) Veterans per room, and with a bed and other furnishings such as a dresser, storage, and personal care items. Residents must be assisted in maintaining an acceptable level of personal hygiene and grooming. The environment must be conducive to social interaction, and supportive of the recovery model and of the fullest development of the Veteran s rehabilitative potential. A minimum of three (3) nutritionally adequate meals per day must be provided in a setting that encourages social interaction. In addition, Contractor must make available snacks of nourishing quality (e.g. fruits, vegetables, protein sources) between meals and before bedtime for those requiring or desiring additional food, whether or not it is medically indicated Contractor must provide laundry facilities and necessary detergent free of charge and adequate for Veterans to do their own laundry or to have laundry done at minimum one time per week. Contractor will assist the Veteran to access public transportation, including providing information and instructions necessary to enable Veterans to utilize public transportation. If public transportation is inappropriate or inadequate, Contractor will assist Veteran to identify alternative transportation available to accommodate Veteran s needs. Contractor shall provide professional counseling and clinical interventions in an individual or group setting, as appropriate, which may include self-care skills, adaptive coping skills, vocational rehabilitation, Cognitive Behavioral Therapy (CBT), Mind Body Stress Reduction (MBSR), Motivational Interviewing (MI), psychoeducational tools, sober living skills, psychotropic medication education, disease process/recovery education, or other clinically relevant interventions.
