Description
Tunnel Flooring Replacement VA Salt Lake City Health Care System SLC Tunnel System GENERAL Scope of Work The VA Salt Lake City Health Care System (VA SLC HCS) has a requirement for all labor, material (floor prep, adhesive and sealer only), equipment, travel, and supervision necessary to remove existing flooring and install VA stocked Floorazzo tile using the wet set installation. This requirement includes the developing a phasing plan with coordinating with designated COR. Utilizing Weekends and after hours to accomplish work to minimize the disruption of patient traffic, in Tunnel A and Tunnel B . Tunnel C shall be during regular work hours. Performance Requirements The specification covers the requirement demolition of existing flooring, floor prep, and install of VA stocked Floorazzo tile. The minimum acceptable requirements for manufacturer s extent of project line, design, materials, workmanship, performance, safety and services are set forth, hereinafter. Failure to meet the minimum acceptable requirements may result in disqualification of the bid. Normal Duty hours will be 8am- 4:30pm M-F. The Contractor Shall: Develop an approved phasing plan with coordinating with the COR. Utilizing Weekends and after hours, as needed to complete work done in Tunnel A and Tunnel B . Tunnel C shall be completed during normal business hours. This plan needs to show how disruption to mission, employees, and patients is minimized. Phased plan(s) must be received in writing and approved by COR a minimum of ten (10) working days prior to work. Provide Infection Control Risk Assessment paperwork in accordance of VA SLC HCS policy. The Contractor shall be made aware of and must comply with local security policies and procedures in the task order. The Contractor will comply to these standards during all site visits on Governmental property. Contractor shall contact the VA COR at least 14 days prior to work start date to ensure compliance to any local security policies. The Contractor personnel shall present a professional appearance and shall have a uniform or some method of identifying them as an employee of the contractor. A VA badge will be provided and must be worn above the waist and be visible at all times while on government property. The Contractor shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental risks. The Contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, Building Codes, Life Safety Codes, Occupational Health and Safety Administration (OSHA), ADA/ADAG, permits, VA design standards (www.cfm.va.gov/til/dguide.asp), and instructions required for the performance of the duties in the Statement of Work (SOW). The Contractor is responsible for ensuring that its firm and staff maintain any generally required professional certification, accreditation, license, bond, and proficiency relative to their area of expertise. The Contractor shall retain documentation of such records. The Government will not pay for expenses to meet this requirement. The Contractor is responsible for ensuring staff are trained and certified for planning and installing product to maintain the integrity of the manufacturer s warranty. Contractor must be able to provide valid certification documentation of all installers at the time of installation start. Or Contractor shall hire the manufacturer s representative to provide a minimum of two (2) days of training and onsite supervision. Training shall be conducted prior to the start of the installation. Installing company shall provide installers ample time to participate in the training and certification process. This shall be included in the price of the installation from the contractor. Provide a local Project Manager/Lead Installer to be physically onsite during Installation and managing installation with a cell phone for communication. Provide a Certified Floorazzo Flooring Installation consultant to consult prior, during, and after Installation. Contractor to provide a testing agency that is independent and qualified to perform concrete substrate moisture and humidity testing according to AST200°F ( 93°C) Product to have a minimum shelf life of 2 years when stored in original unopened packaging at 73° F Product shall be ready for heavy traffic in less than 24hours Product have a coverage of 185 to 245 Square Foot per US gal. Contractor to install product per manufacturer s instructions. Product to be ready to use with no mixing necessary. Product to withstand indentation from heavy static and rolling loads for golf carts and other hospital carts that roll through the tunnel. Sealer Contractor to use manufacture s recommended sealer. Sealer to be compliant with California 2010 VOC requirements Sealer to be applied with 3-5 coats using a clean synthetic finish mop and bucket. Sealer to be self-leveling and have a gloss finish. Sealer to have a maximum 45 minutes drying time per coat. Sealer to have >200-degree F flash point. Sealer to have 8.8 PH level. Sealer to have a minimum of 2-year shelf life. Sealer to have a minimum of 0.5 Slip resistance (ASTM D2047) Sealer to have a maximum of 19% solids Sealer to have a maximum of 1% VOC VA supplied Terrazzo Tile (For information only) Use VA supplied Terrazzo Floor tile Tile product made with polyester resins. Product is 2 x 2 Tile and shall be installed in butt tile fashion, where tiles are laid next to each other without grout line. Performance: Physical properties of Floorazzo Tiles shall conform to the following minimums: Safety Critical radiant flux ASTM E648 Class 1 Smoke density ASTM E662 Pass Slip resistance ASTM C1028 Wet: 0.7; Dry: 0.66 Durability Static load limit ASTM F970 1,000 psi Chemical resistance ASTM F925 No change LEED-NC v.2.2 MR Credit 4: Recycled content Gem (Glass): 18% pre-consumer recycled content 18% post-consumer recycled content Marble: 18% pre-consumer IEQ Credit 4.1: Low-emitting adhesives Mapei Ultrabond ECO 373 Adhesive has a low VOC content IEQ Credit 4.2: Low-emitting sealers Diversey Plaza Plus Sealer/Finish or equal IEQ Credit 4.3: Low-emitting flooring Floorazzo is manufactured with a mineral base EXECUTION Contractor Supervisor: A competent and experienced English-speaking Contractor Supervisor shall be provided by the Contractor whenever work is being performed. The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. This includes the requirement that the contractor have a Competent Person on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. Documentation of these requirements shall be submitted and approved prior to performing the work. The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. This includes the requirement that the contractor have a Competent Person on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. Documentation of these requirements shall be submitted and approved prior to performing the work. INSPECTION Prior to work of this Section, carefully inspect area. Verify all such work is complete to the point where this installation may properly commence. Prior to ordering materials field verify all dimensions. Verify that work of this section may be installed in strict accordance with the original design, all pertinent codes and regulations, and all pertinent portions, of the referenced standards. In the event of discrepancy, immediately notify the Contracting Officer Representative, (COR). Do not proceed with installation in areas of discrepancy until all such discrepancies have been fully resolved. Overall: Follow guidelines laid out in Division 01, Section 01 71 00 Examination and Preparation as well as Section 01 43 00 Quality Assurance. Verification of Conditions: Inspect all substrates and subfloors for proper tolerances and dryness and report any discrepancies to the general contractor in writing. Pre-installation Testing: Verify that concrete testing per ASTM F710 has been conducted by an independent testing agency, and that results are within the adhesive and floor covering manufacturers requirements. Evaluation and Assessment: See the state requirements for the project location. SURFACE PREPARATION Follow guidelines laid out in Division 01, Section 01 71 00 Examination and Preparation. Prepare concrete substrates per ASTM F 710. All work required to put the concrete subfloor in acceptable condition shall be the responsibility of the general contractor. Must adhere to manufacturer requirements for acceptable floor levelness and preparation. INSTALLATION Coordinate installations per construction schedule. Coordinate install date with COR. All installation methods shall be in accordance with the printed instructions of the manufacturer. Installation Instructions All items must be placed in the proper location as indicated on the Floor Plans. Each item must be clean and free from dust, packing marks, tags, labels, etc. Proper storage and acclimation of product according to manufacturer s procedures, including letting the product sit flat in installation area for recommended time period from manufacture. Follow Division 01 relevant guidelines, and the latest edition of the manufacturer s installation instructions (available from www.floorazzo.com). Only use written instructions from the manufacturer Interface with Other Work: Transitions are required to and/or from the specified floor covering, use Schluter VinPro- T or equal transition to interface with other existing floor covering at the same height (https://www.schluter.com/schluter-us/en_US/Resilient-Surface-Coverings---LVT/Profiles-for-Resilient-Surface-Coverings-/For-Floors/Schluter%C2%AE-VINPRO-T/p/VINPRO_T ). FIELD QUALITY CONTROL Field Tests/Punch walk with VA COR after completing each phase for final acceptance by VA COR. The flooring shall be neat in appearance and free from defects. Manufacturer Services: Coordinate with supplier manufacturer s representative. As stated in section: 1.3.3.8. Installers without prior experience must have manufacturer s representative on site for training and oversight as required by the manufacturer. Protection of walls, and ceiling is the responsibility of the installers during installation. Any damage shall be replaced with like material and finish at no cost to the Owner Installers must comply with VA vendor rules and guidelines including wearing proper attire. Installers must check in with VA COR to receive VA vendor badges. WORK STANDARDS AND ICRA/ILSM Work site shall be free of debris and returned to original condition upon completion of demolition and construction, preventative maintenance, or repairs. All work is to be performed in areas used daily by facility clients and staff. Contractor will take suitable measures to control all material and equipment to produce a minimum of interference in facility routine. All work shall be performed in a safe and prudent manner. Contractor shall so perform all work as to offer the least possible obstruction and inconvenience to staff and the public and shall have under construction no greater amount of work than can be completed promptly and properly with due regard to the rights of staff and the public. Any person the COR may deem incompetent or disorderly shall be promptly removed from the facility and not be allowed to return for the duration of the agreement. The Contractor shall comply with an Infection Control Risk Assessment (ICRA) which will be developed with the COR and the Infection Control Practitioner. Multiple ICRA s may be necessary to address specific risks at various stages of the project and must be approved prior to proceeding on each phase. CLEANING General: Maintain areas free of waste materials, debris, and rubbish. Maintain site in a clean and orderly condition. Clean up job site, including sweeping or dust mopping the floor to remove all dirt or grit, and put all waste in general contractor s dumpster. Initial Maintenance: Installer is responsible and must conduct the initial penetrating floor sealer and surface treatment. It must be conducted immediately after installation. Use cleaning materials which do not create hazards to health or property and which will not damage surfaces. Use only those cleaning materials and methods recommended by manufacturer of the surface material to be cleaned. Use cleaning materials only on surfaces recommended by cleaning material manufacturer. Provide for all dumpsters, haul fees and dump charges as required. Do not use Owners collection facilities at any time. ACCEPTANCE STANDARDS The VA reserves the right to dispute completion of contract scope if contract work is found not in accordance with manufacturer recommendations, is not satisfactorily executed, or if the system is not properly tested and inspected to achieve certification. All work completed by the Contractor must be reviewed and approved by the VA. PUNCHLIST INSPECTION Punchlist inspection will not commence until the above items are complete and ready for inspection. If the Contractor schedules an inspection and the above items are not completed, the Contractor will be responsible for any and all additional costs incurred by the Owner s representative, including but not limited to travel costs and professional fees at the Owner s representative s hourly rate. PERIOD OF PERFORMANCE Government intends to issue Notice to Proceed (NTP) with Contractor. The period of performance for this contract is two hundred eighty (280) calendar days.
