- The VA Caribbean Healthcare System (VACHS), 10 Casia St, San Juan, PR 00921, has a requirement for health and safety inspections. These services will be provided at the VACHS main facility, as well as its outlying facilities, which are: the Mayaguez Out-patient Clinic (MOPC), Ponce Out-patient Clinic (POPC), Arecibo Out-patient Clinic, Guayama Community-Based Out-patient Clinic (CBOC), the Comerio, Utuado, and Vieques Rural Clinics, as well as the St. Croix and St. Thomas Out-patient Clinics. The Contractor shall provide all management, supervision, personnel, materials, equipment, and services necessary to conduct an annual health and safety external inspection to enhance and maintain the VACHS organization s health and safety practices, to comply with the Commission on Accreditation of Rehabilitation Facilities (CARF) at each of the facilities mentioned above. North American Industry Classification System (NAICS) Code for this requirement will be 541690 (Other Scientific and Technical Consulting Services), and the Product Service Code (PSC) will be H399 (Inspections Miscellaneous). All responsible sources may submit a capability statement with sufficient detail to demonstrate the ability to meet the subject requirement. This is not a solicitation. If there is sufficient interest, a solicitation shall be posted on the FBO website. Interest/capability statements shall be submitted to Wilfredo Perez via e-mail at wilfredo.perez3@va.gov by April 17, 2023 at 10:00 AM Atlantic Standard Time (AST). Telephone inquiries will not be accepted. In order for a vendor to be found to be capable of fulfilling the Government s requirement, he shall demonstrate the following: That he has a minimum of five (5) years of experience providing occupational health and safety services consulting services in the healthcare industry for compliance with National Fire Prevention Agency (NFPA), TJC standards, OSHA 1910, and CARF in hospitals. That he has a minimum of five (5) years of experience in providing comprehensive health and safety inspections to the Department of Veterans Affairs complying with CARF 1.H.15. Prospective offeror shall also demonstrate that he possesses at least one of the following: Occupational Health and Safety Management System (OHSAS) 18001 / ISO 45001:2018 certified with a minimum of five (5) years of experience in the healthcare industry field. Quality Management System ISO 9001:2008 certified with a minimum of ten (10) years of experience in the healthcare field, shall be Environmental Management System ISO 14001 certified with a minimum of five (5) years of experience in the healthcare industry field, and shall be a Safety Specialist/Safety consultant knowledgeable to the health and safety requirements applicable to the services requested with a minimum of five (5) years of experience in the healthcare industry. Certified Industrial Hygienist (CIH) from the American Board of Industrial Hygiene (ABIH) with a minimum of five (5) years of experience in the healthcare industry field. Certified Safety Professional (CSP) with a minimum of five (5) years of experience in the healthcare industry field. Licensed Fire Protection Engineer (PE) with a minimum of five (5) years of experience in the healthcare industry field. Licensed or registered Safety Engineer with a minimum of five (5) years of experience in the healthcare industry field. Safety Management Specialist (SMS) certified with a minimum of five (5) years of experience in the healthcare industry field. Safety Specialist/Safety consultant knowledgeable to the health and safety requirements applicable to the services requested with a minimum of six (6) years of experience in the healthcare industry. Please provide the following information as an attachment to your response: 1. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services under the same or a related NAICS codes. 2. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. 3. Do you plan on responding to a solicitation for this requirement with a Joint Venture utilizing multiple owned companies as majority or non-majority owner? Yes ___No ___ If yes, please identify which companies are considering a Joint Venture and the ownership of each company.
