Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research, to make appropriate acquisition decisions, and to gain knowledge of potential qualified businesses capable of providing the following: Manufacturer for Dental Base + 4 Option Years for the Dwight D. Point of Contact for this Sources Sought is: veronica.flores@va.gov (913) 946 - 1167 Department of Veterans Affairs Network Contracting Office -15 3450 S. 4th Street Trafficway Leavenworth, KS. 66048. STATEMENT OF WORK (SOW) FOR Dwight D. Eisenhower VA Medical Center General: Contractor shall provide all labor, material, and equipment necessary to fabricate full and partial acrylic dentures with clasps and other miscellaneous services for: Department of Veterans Affairs VA Medical Centers (VAMC): Dwight D. Eisenhower VAMC 4101 S 4th St. Leavenworth, Kansas 66048 Pick-up and Deliveries: Contractor shall be required to make daily pickups and deliveries (Monday thru Friday) to the facilities. The contractor may satisfy this requirement by making a daily phone call to the Dental Department between the hours of 3:00 PM 4:00 PM local time to determine if a contractor pickup is required. All materials delivered shall have the patients' last name and only last four digits of social security number affixed to it. Denture Services: 1. Contractor price for a finished denture shall include the following: Custom impression trays for secondary impressions. Box and pour master cast Record base with occlusal rims Articulate and set-up teeth in wax for try in. Finished denture remount and check and adjust occlusion as indicated. 2. Contractor shall be required to use Lucitone 199 or equivalent product. 3. Processing of dentures shall be by heat cure technique. 4. Finished dentures will be evaluated for the following: Balance occlusion, or as requested by doctor. Aesthetics. Contour and polish. Denture teeth will be supplied by the VA or will be supplied by contractor for fee. 5. The VA will provide cast partial framework for removable partial dentures. 6. Contractor shall not alter borders. Miscellaneous Services: Contractor shall provide the following: 1. Pour, pindex, and base crown and bridge dies. Solid cast as indicated. 2. Repairs dentures and removable partial dentures (RPD) 3. Reline dentures and removable partial dentures (RPD) 4. Fabricate custom impression trays. 5. Construct surgical and occlusal splints. 6. Improve contact in fixed cases, (Crown or fixed partial dentures). Adding porcelain or gold solders in full cast restorations. Quality of the Product: The government requires a finished denture to be of a higher quality than a typical ""economy type"" denture. Delivery Schedule: The following delivery schedule is required for the below listed procedures. All delivery times are based on Lab workdays. You must be able to abide by the delivery times to be considered as capable manufacturers. Procedure Delivery Time Estimated Quantity Crowns, Bridges 7 days 25 units / month Implant Prostheses 7 days 5-10 units / month Partial to completion 10 days 5 / month Nightguards 5 days 5 / month Denture Finish 5 days 5 sets / month Denture Relines 5 days 4 / month Diagnostic wax-up 5 days 2 / month *Example: If the contractor makes an on-site pick-up for denture repair on Monday, the Government requires on-site delivery of the repaired denture no later than the close of business (4:30 PM) on Wednesday. Monthly Invoices: All monthly invoices shall be sent electronically. Monthly invoices shall provide an itemized listing of all appropriate charges, the designated contract number, obligation number and vendor tax ID number. This is a base +4-year contract. Disinfection: 1. All impressions and other items that have contacted oral tissues will be properly disinfected prior to the sending it to dental lab. Rinse and remove gross bioburden, disinfect with ADA approved surface disinfectant, Cavicide 1. 2. All materials returned to the VA shall have been disinfected by use of an ADA approved chemical disinfectant
