AMENDMENT 0003:� The purpose of amendment 0003 is to incorproate the following: 1.��Responses to a question received by the contracting office, see below. Questions #7:� Can you provide the current contract number and company name of the incumbent contractor?� Response #7:� The previous contracting vehicle was a Blanket Purchase Agreement (BPA).� The BPA number was W81K0218A0018.� The contractor was Immucor, Inc.�� ********************************************************** AMENDMENT 0002:� The purpose of amendment 0002 is to incorporate the following: 1.��Responses to a question received by the contracting office, see below. Question #6:��Can you accept the delivery service of APO or a private delivery service from the USA manufacturer directly to the Ship to address specified in the CLIN, if it will be possible for cold chain delivery condition? Response #6:� Delivery of APO cannot be accepted.� Only private delivery service can be accepted.� Cold chain delivery is required.� See required storage temperatures in the revised Attachment 0001 Minimum Requirements.� � *********************************************************** AMENDMENT 0001:� The purpose of amendment 0001 is to incorporate the following: 1.� Change the Unit of Issue from JOB to LOT for Contract LIne Item Numbers (CLINs) 0001, 1001, 2001, 3001 and 4001.� See� revised solicitation document W81K0223Q0027_Amend0001w2Attachments. 2.� Responses to questions received by the contracting office, see below. Question #1. Unit Job, Page 1 of 76 , Please explain what is �Job� for Unit.� Is it 35 ea (all items for item 1 through 35) on Attachment 1? Response #1:��Amendment 0001 has changed the unit of issue from JOB to LOT.� LOT is the unit of issue that will include all items in either the contract line item and/or the delivery order line item.� Estimated and Maximum Quantities for each item are listed in Attachment 2.� Since the resulting contract will be an Indefinite Delivery/Indefinite Quantity type contract, actual quantities delivered, and the�frequency of the deliveries will be reflected in the delivery orders issued.�� Question #2:� Your requirement is 2 Job for base year and 4 Job for all of option year, how to make the unit price for 1 Job? Response #2:��The contract line item number (CLIN) Extended Price should include all the items listed on Attachment 2 of the solicitation.� In Attachment 2, the Unit Price of each item should be based on the maximum quantity.� The extended price should be calculated using the maximum quantity not the estimated quantity.� Then the total of all items in the base period should be indicated in CLIN 0001 as the Extended Price on page 1. Finally, divide the Extended Price by the quantity to get the unit price. Apply the same procedure to calculate the Unit Price for each option. Question #3: Your requirement is that all of reagents shall be US FDA approved.� Can you accept some items which is approved by EU or Korean Government instead of US FDA? Response #3:� All items must be approved by the United States Food and Drug Administration (US FDA).� Question #4: �How many minimum order quantity for the reagent is? Response #4:��The minimum guarantee is $5,000. The quantity of each item listed on Attachment 2 will depend on the actual need.�� Question #5:��Can you purchase one(1) item only of 35 reagents at a time? Response #5:��All�cell reagents including QCs with short shelf life will be delivered approximately every 4 weeks.�Other consumables are anticipated to be scheduled every other month and ANTI-e & evaporation caps will be�needed approximately once a year.� Exact quantities and the timing of their shipments will be coordinated as indicated on the� Delivery Order.�� 3.��All questions regarding this requirement/solicitation shall be submitted to the contracting office no later than Friday, March 3, 2023 10:00 am (HST). ********************************************************** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Medical Readiness Contracting Office - Pacific, Bldg 339 146 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and award an Indefinite Delivery/Indefinite Quantity type contract with Firm Fixed-Prices for Blood Bank Reagents for the Brian D. Allgood Army Community Hospital, Korea.� This is a request for quotation (RFQ). The solicitation number is W81K0223Q0027.� �The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01. �The Standard Industrial Classification Code is 6550, size standard is 1250 employees and the NAICS code of 325413.� Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. Provision at 52.212-2 Evaluation - Commercial Items and its addendum applies to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition.� FAR clauses that are applicable to this acquisition are in the attachment to the combined synopsis/solicitation. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award an Indefinite Delivery/Indefinite Quantity type contract with Firm Fixed Prices for blood bank reagents to the successful quoter.
