Description
STATEMENT OF WORK PART A GENERAL INFORMATION - A.1 INTRODUCTION This requirement is to replace 4 each storefront automatic doors and reuse their frames and glass. A.2 BACKGROUND New automatic access with locking ability is needed for Tower entrance. Existing automatic doors are beyond economic repair and needs to be replaced. Work will be done at 1101 Veterans Drive, Lexington, KY 40502. A.3 SCOPE OF WORK The Contractor shall furnish all labor, supervision, and material to provide new all new equipment, connect to existing PACs system, and test for proper operation. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1 Tasks to be accomplished: Remove 4 sets of automatic doors and rework existing frames and glass to accommodate new doors and operators. Install 4 pair of Dark Bronze, Horton brand, overhead concealed door packages with LZR Microscan safety sensors and 3 guardrails. Program and verify proper operation. Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. B.2 Tasks by others: VA Staff will disconnect and reconnect the 120v circuits. Additional tasks: The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by VA which are generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the task order. The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the Contracting Officer for response. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer. No contractor access to VA computer systems required. PART C SUPPORTING INFORMATION C.1 Place of Performance Troy Bowling Campus, Lexington VA Health Care System, 1101 Veterans Dr. Lexington, KY 40502 C.2 Work Hours During normal business hours 8:00am thru 4:30pm. C.3.1 Contractor Furnished Materials All labor, materials, equipment to perform the job. When ""furnish"", ""provide"", ""install"", or similar term is used it shall mean a complete installation, ready for use. C.3.2 Government Furnished Materials and Services. Government furnished property, if any, will be identified and will only be used for the performance of this contract unless otherwise authorized by the Contracting Officer. The Contractor shall, always, take all steps necessary for maintenance and preservation of all Government-furnished property. The Contractor shall comply with all reasonable requests of the Contracting Officer to enclose, or specifically protect, Government-furnished property. C.3.3 Qualifications of Key Personnel Contractor s employees should be knowledgeable of standards associated with this type of work. C.3.4 Supervision/Communications. Contractor shall coordinate service call with Wade Sparks (VA Electrical Supervisor) upon arrival to the facility. Contact information 1 (859) 230-5042 C.3.5 Security Requirements: The Authorization requirements do not apply, and a Security Accreditation Package is not required. Contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall be always worn above the belt while on Medical Center property. Contractor's employees shall return all I.D. badges to designated personnel at time designated in the task order. C.3.10 Smoking Policy. Smoking is prohibited on Federal property. PART D PERIOD OF PERFORMANCE D.1 The Period of Performance for this requirement is To be Determined (TBD) Days from date of award.
