Description
Please submit technical capability (UEI/SAM # etc...), letter of intent (email or memo stating that you can handle the job), active SAM registry and socio-economic status (8a, hubzone,sdvosb, small business etc..) Please send all inquiries to Addlene.Williams@va.gov by the stated close-out date. Statement of Work Contractor shall provide laboratory services to include specimen packaging and transport of specimens to its laboratories; pre-analytic processing as defined in its Laboratory Testing Manual; performing test for Thyroseq molecular testing for SFVAMC patients. After the sample is collected from the patient and received in the Anatomic pathology laboratory, the container is labeled with patient name, accession number along with the completed requisition form with patient information and type of tests. The correct patient�s name, accession number and proper tests are always verified by SFVAMC staff for each request. Clinical molecular Thyroseq testing is performed to determine if surgery is appropriate for abnormal thyroid nodules. Results from this will frequently determine a patient has low risk of malignancy and the patient will not have to undergo surgery. This test is important in preventing overtreatment, avoiding potential surgical complications, and saving medical center resources. Place of Performance:� Services shall be provided at the outside laboratory. The SFVAMC pathologists determine whether Thyroseq testing is needed based on the amount of tissue available, and the tissue diagnosis rendered from tissue evaluation. The total monthly cost depends on number of specimens received within the pathology laboratory which is average 1-3 cases a month based on review of our thyroid biopsy records from the past two years. Contractor shall be responsible for all the work. At any time during the term of the contract, the Contractor changes their subcontractor, the Contractor shall provide the Contracting Officer the name of the new subcontracted laboratory, including the list of tests subcontracted to be performed at that laboratory.� Any testing performed by a subcontracted laboratory must be clearly stated on the report, either electronically or printed report. B.������ Licensing, Accreditation, and Inspection of Contractor: 1.������ The Contracting Officer reserves the right to thoroughly inspect and investigate the Contractor's facilities and other qualifications. 2.������� The Contractor and all subcontracted facilities shall have all licenses, permits, accreditation and certificates required by law. Current copies of these must be provided to the Contracting Officer upon request. The Contractor shall be accredited by the College of American Pathologists (CAP). The Contractor shall be accredited by all other accreditation/regulatory agencies, as appropriate. The Contractor agrees to furnish the Contracting Officer with a copy of current accreditation certificate(s) and accreditation number(s) for the Contractor's laboratory and all sub?contracted laboratories. 4.������� Contractor's Laboratory Director shall be a licensed pathologist. C.� Test Results. 1.� ����� Routine.� The Contractor shall provide routine test results to the ordering SFVAMC within 2 to 3 weeks of specimen receipt, except when specimens are picked up the day before a weekend or holiday (in which case the results will be provided the following business day.� 2.� ����� Telephone Consultation.� The Contractor shall consult with the SFVAMC COTR, requesting Clinician or designee at the SFVAMC by telephone as needed. 3. ������ The Contractor shall immediately telephone requesting Clinician or On Call Provider to report Critical Values or test result that may indicate a life threatening condition. D. Quality Control: To ensure proper handling and test performance, the Contractor shall provide the Contracting Officer or his/her designee with the following updated information with the proposal as well as during the term of the contract upon request: 1.� ����� For quality purposes, tests routinely performed with the stained slides and original blocks. 2.������� Proficiency testing data shall include a list of tests that are classified as unacceptable on the survey evaluation for the past two (2) years. The Contractor shall notify the Contracting Officer or his/her designee of any test that is classified as unacceptable based on Proficiency testing performance and evaluation during the contract period. 3.������ Contractor shall provide address of processing sites under contract. 4.������� Contractor shall provide detailed descriptions of quality control procedures and performance. 5.������� The Contractor's facilities, methodologies (defined as the principal of the method and the references), and quality control procedures may be examined by representatives of the SFVAMC at any time during the life of the contract. 4. Reimbursement Method: Per-Procedure. Refer to VA directive 1663 for specific Guidance related to acceptable price schedules for on-site and off-site services. (Note: Please note that if the healthcare resource being procured involves procedures Being performed at a VA facility, as approved by the VISN Director, the rate of Reimbursement should exclude the practice expense RVU and reimburse for the work And malpractice component of Medicare Part B only.)
