- The United States Department Of Naval Medical Research Center(NMRC) intends to�award a sole source contract, Roche Diagnostics Corp.,�Indianapolis, IN, 46250.�This notice is not a request for competitive proposals however If interested Government shall consider all responses. � The NAICS code for this action is 811210. This requirement will be procured under the guidelines of FAR Part 12 Commercial Items. NMRC requires that all contractors doing business with the Acquisition Office must be registered with the Systems For Award Management (SAM). No award can be made unless the vendor is registered in Systems For Award Management. For additional information and to register in SAM, please access the following website: http://www.sam.gov/. In order to register with the SAM and to be eligible to receive an award from this Acquisition Office, all offers must have a DUN & Bradstreet Number. A Dun & Bradstreet number may be acquired free of charge by contacting Dun & Bradstreet online at https://www.dnb.com/products/eupdate/requestOptions.html or by phone at (800) 333-0505 Maintenance Service Contract for a Base/ 3 Option Years. Please provide pricing for the 3 option years in your quote 1.�Roche MagNAPure 96SN: 4960, Qty1 ea/ 12mos.� SOW:� The Contractor shall provide trained, experienced, English speaking personnel, labor, tools, diagnostic equipment, software, test phantoms, material, supplies, transportation, parts, and equipment necessary to perform Preventive Maintenance (PM), Calibration (CAL), Safety testing (ST) and remedial repairs. The Contractor shall provide a full service report within two (2) days after completion of all service performed.� The service report shall include, but not be limited to: contract number, contractors log number, detailed description of the services(s) performed, replacement parts(s) information (part number, part value, nomenclature, unit price, manufacturer, if not OEM, and whether the part is new/used/reconditioned, the completion date and time, man-hours expended and the hourly rate normally charged for the type of service performed, and the name of the technician performing the service. Normal Working Hour Maintenance Coverage. Maintenance Coverage shall be performed Monday thru Friday, between 8: 00 a.m. to 4: 30 p.m.� The Contractor shall telephonically respond within__24__ business hours after receipt of notification of equipment failure, and shall provide onsite service within�� 2�business days.� Equipment shall be operational within 48 business hours.� The Government reserves the right to deduct from the Contractor�s payment an amount per hour equal to the number of hours the Contractor fails to respond, *as listed above.� Emergency services outside the normal working hours by the Government site shall be billable to the Government at published commercial rates, and negotiated prior to services rendered.�������������������������������������������������������������������������������
