- THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of qualified sources to perform the potential requirement. This source sought is UNRESTRICTED and seeks providers capable of fulfilling the requirement in accordance with the Statement of Work. The responses received from interested contractors will assist the Government in determining set-aside and the appropriate acquisition method. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 20, is conducting market research to identify potential contractor sources which can provide the following services: Annual Preventative Maintenance for eight (8) Steris units for the Puget Sound VA Health Care System in accordance with the attached Statement of Work. Potential sources having the capabilities necessary to provide the above commodities which meet all the requirements outlined in the attached statement of work are invited to respond to this Sources Sought Notice via e-mail to Dane.Gift@VA.Gov no later than noted in the RFI. VAAR 852.212-71 Gray Market Items will be included in the contract as the items are no longer under warranty but need to stay in good standing with the OEM in terms of replacement parts used. No telephone inquiries will be accepted. Responses should include the following information: Company name, address, and business size; point of contact name, phone number, and e-mail address; A capability statement addressing the requirements in the attached Statement of Work to include: References for similar commodities currently being provided or previously provided at the required service levels as outlined in the Statement of Work Capability Statement Capability to provide required supplies at VISN 20 in accordance with the Statement of Work Any Steris certifications or authorized-provider status NAICS Code 811219 is applicable to determine business size standard. Disclaimer and Important Notes: This Sources Sought Notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. The Government will treat any information received as proprietary and will not share such information with other companies. Any organization responding to this Sources Sought Notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to supply the required commodities. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. The Government may or may not issue a solicitation as a result of this announcement. There is no solicitation available at this time. STATEMENT OF WORK (SOW) 1. Background. Puget Sound Health Care System (PSHCS) Seattle Division, Research and Development Section requires on-site, preventative maintenance contract to ensure that the building 101 Steris Glass Washers and Autoclave equipment and the building 1 Autoclaves receive the manufacturer s recommended preventative maintenance on the manufacturer s recommended schedule to ensure the equipment is ready for safe and effective use. This contract must include all labor, parts and service needed for quarterly preventative maintenance to ensure that all units are fully operational according to manufacturer s preventative maintenance specifications. 2. Scope. Contractor shall provide all necessary travel, labor, materials, tools, documentation, and parts required for the performing of quarterly scheduled preventative maintenance for the Seattle Division Research and Development Building 101 Glass Washer and Autoclaves and Building 1 Autoclaves. Preventative maintenance shall be provided at a prearranged time between 8:00 am to 5:00 pm, Monday through Friday, excluding federal holidays. 3. Specific Tasks. The contractor shall return the equipment to the operating condition designed by the manufacture The contractor shall furnish a completed service report indicating the result of the scheduled inspection 4. Performance Monitoring 4.1. The contractor shall provide the RD Lab Operations Manager Scott Ng-Evans or the COR with a completed Preventative Maintenance service report for all work performed. 4.2. The progress report shall cover all work completed during the preceding quarter and shall present the work to be accomplished during the subsequent quarter. This report shall also identify any problems that arose, along with a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved, with an explanation. 4.3. For unresolved problems that require repair or replacement parts a quote for the repair cost will be provided. Repair work is outside of the scope of this contract. 4.4. The tasks are well defined and can, therefore, be easily monitored by the RD COR and RD POC, who shall represent the Government for this process. Contractor performance will be monitored and the work certified by the COR. The COR will assure contractor quality by routine inspections, product sampling, and monitoring of work while work is being performed. 5. Security Requirements The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall have intermittent access only and will be escorted and supervised by VA employees while at VA Facilities. No background investigation is required. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Government furnished property will not be provided to the contractor. 7. Other Pertinent Information or Special Considerations. 7.1. Identification of Non-Disclosure Requirements. It is agreed that: 7.1.1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 7.1.2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 7.1.3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. 9.2. CONTRACTOR EXPERIENCE REQUIREMENTS 9.2.1. Each respondent shall have an established business, with an office and full time staff. The staff includes a ""fully qualified"" Field Service Engineer (FSE) and a ""fully qualified"" FSE who will serve as the backup. ""Fully Qualified"" is based upon training and on experience in the field. For training, the FSE(s) has successfully completed a formalized manufacturer training programs within the last two years, for the equipment identified in the equipment schedule, and annual refresher course. For field experience, the FSE(s) has a minimum of two years of experience, with respect to scheduled and unscheduled preventive and remedial maintenance, on equipment listed. The FSEs shall be authorized by the contractor to perform the maintenance services. All work shall be performed by ""Fully Qualified"" competent FSEs. The contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the contractor services at the PSHCS. The CO may authenticate the training requirements, request training certificates or credentials from the contractor at any time for any personnel who are servicing or installing any PSHCS equipment. The CO and/or the COR specifically reserve the right to reject any of the contractor's personnel and refuse them permission to work on the PSHCS equipment. Subcontractor will not be used under this contract unless authorized and approved by the COR. CHANGES TO STATEMENT OF WORK 9.3.1 Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. 10. Risk Control The equipment covered under this PM service contract is required to ensure that all units are maintained according to manufacturer specifications. The equipment ensures that the lab ware, glassware & instruments used in research laboratory procedures are safe for use. 11. Place of Performance. Work will primarily be performed at the Department of Veterans Affairs Puget Sound Health Care System Seattle Division 663 Building 101, Room 3W91 Building 1, Rooms 601, 603 and 618 1660 S. Columbian Way Seattle, WA 98108-1597 12. Period of Performance. The contractor shall complete the work required under this SOW in once given the award for and then for a year 4 Quarterly visits, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. Proposed Period of Performance: A base year plus 4 option years Base Year: January 1st 2023-December 31st 2023 Option Year 1: January 1st 2024- December 31st 2024 Option Year 2: January 1st 2025- December 31st 2025 Option Year 3: January 1st 2026- December 31st 2026 Option Year 4: January 1st 2027- December 31st 2027 Any response to this notice must show clear and convincing evidence of the Vendor s capability of meeting this requirement as found above (1-4). Please note, responses will be shared within the Government and project team, but otherwise will be held in strict confidence.
