Description
This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information only. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The Department of Veterans Affairs Harlingen Medical HealthCare Center Ophthalmology/Optometry Service in Harlingen, Texas intends to award a brand name or equal contract award for the purchase of Patient Mobile Bed. The Government is conducting a market survey to help determine the availability and technical capability of qualified service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, HUBZone small businesses and/or other large businesses capable of serving the needs identified below. This notice of intent is for open market as well as Federal Supply Schedule items. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business, relative to NAICS 339113 with a size standard of 750 employees. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov or GSA E-Buy at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 3.00 EA MOBILE PATIENT BED ASSY, RFID & MPB RF DOOR ASSY, POWER CORD MOBILE PATIENT BED ADDENDUM, US EN, BATTERY UNIT, MPB GRAND TOTAL Statement of Work General: Surgery Mobile Patient Bed Description of Work: This statement of work defines the effort required for VA Texas Valley Coastal Bend Healthcare System (VATVCBHCS), Ophthalmology Section of Surgical Service to procure 3 complete additional optometry mobile patient beds for one (1) location. Harlingen is the location requesting in the optometry department in order to facilitate the increased demand for patient optical care in order to service veterans in a timely manner. Proposed mobile patient beds will allow for the maximization of space due to limited available space within the clinics. Place of Delivery - The vendor shall deliver the described herein to: VATVCBHCS VA Harlingen Healthcare Center (HCC) Receiving Dock 2601 Veterans Drive Harlingen, TX 78550 POC: TBD Alt POC: TBD Alt POC: TBD Product Requirements for Mobile Patient Bed (3 qty) The patient mobile bed must include a seat, backrest (with the capability to adjust from 0° to 80°), headrest, footrest (with the capability to adjust from 0° to 75°), armrest to include release knobs, lock foot pedals, base, casters (front and back), control pendant and holster, joystick control, din rail (with ability to bear a weight up to 40lbs), auxiliary buttons, and emergency stop button. Patient mobile bed must be ideal in the administration of corneal topography measurements and alignment of corneal surgical procedures as will as for optimal positioning for creating incisions to improve intraocular lens placement. The mobile bed must fall in the sizing of approximately 6ft 5in in length, 2ft 5in in width, with a max weight capacity of approximately 400lbs. Documentation: Contractor shall furnish a delivery manifest once all equipment has been delivered. Payment will not be processed until all equipment has been received by Opthalmology Service. Payment will not be processed until a properly completed service report is received. The service report shall contain, at a minimum, the following information: Date and time of contractor s arrival on station Complete list of instruments and consumable delivered/required The service report shall itemize every item in the specification. Each item shall state a complete description of all work performed concerning the items. Invoicing: Payment to be made by certified invoices and must contain the contract number in addition to the requirements detailed in 52.212-4 (G) to be considered valid. All invoices shall be submitted to the VA Financial Service Center and emailed to the CO. All invoices will reference the purchase order number assigned to the contract.
