- THIS IS A SOURCES SOUGHT REQUEST TO DETERMINE IF NATIVE-OWNED ENTITIES ARE AVAILABLE FOR THE ULTRASOUND SERVICES TO CASS LAKE, RED LAKE AND WHITE EARTH INDIAN HEALTH SERVICES HOSPITAL IN ACCORDANCE WITH THE ATTACHED STATEMENT OF WORK.� THIS IS NOT A REQUEST FOR PROPOSAL.� SEND INTERESTED PARTY INFORMATION TO THE POINTS OF CONTACT LISTED ON THIS SOURCES SOUGHT NOTICE.� STATEMENT OF WORK Diagnostic mobile Ultrasound Red Lake, Cass Lake and White Earth Indian Health Service Hospitals I.� Purpose of Contract:� The purpose of this contract is for the provision of medical imaging services for the Indian Health Service.� The Cass Lake, Red Lake and White Earth Service Units require contractor support from entities to perform medical ultrasound examinations within the Cass Lake, Red Lake and White Earth Hospitals on a part-time basis per facility. The contractor shall furnish the necessary equipment, qualified personnel, and technology to provide quality services to the hospital in compliance with TJC accreditation standards.� The Indian Health Service requires that ultrasound imaging services continue without interruption during the term of this contract. This is a non-personal services contract. II. Objective/description of need: a.� � To provide the patients at the Cass Lake, Red Lake and White Earth Service Units with proper diagnostic ultrasound imaging to assist in quality patient care. III. Government Furnished Property, Facilities, and Services The Service Unit shall provide the space for the contractor to conduct these ultrasound services.� Ultrasound services shall be provided in the Radiology Departments of the Cass Lake, Red Lake and White Earth Hospitals, in an examination/procedure room.� This environment complies with TJC life safety standards. The Service Unit will provide the electronic technology for digital transmittal of images.� The Service Unit will provide the required clerical/reception services required to make appointments, to maximize efficiency, and to ensure a smooth flow of patients.� This includes the distribution of prep information to patients and mailing reminders to patients within three to four days of the scheduled examination. The Service Unit will handle all third party billing.� No other charges shall be made to the patient or the patients� third party coverage. IV. Contractor Furnished Equipment, Facilities, and Services The Contractor shall supply ALL equipment, technologist, gels, transvaginal disinfectants, contrast solutions, tech-sheet paperwork and SDS paperwork.� V. Statement of Work: a. The contact persons shall be: I. Performance Conditions: The Contractor shall provide the equipment needed to perform these ultrasound studies.� The Contractor shall transmit DICOM digital images directly into the Service Units PACS system and Teleradiologist Group PACS system. Contractor will pay for the cost of transporting the equipment, Contractor staff and supplies to and from the Service Unit. Contractor shall supply an ARRT/ARDMS licensed, registered and certified Technologist(s) who will provide these imaging services within the Radiology Departments of the Cass Lake, Red Lake and White Earth Hospitals. The Ultrasonographer will perform the following examinations:� thyroid, breast limited, breast complete, �abdomen complete, renal, bladder, gall bladder/ruq/single organ/limited, ob complete, ob 1st trimester, limited ob, twins 1st trimester & 2nd trimester, biophysical profile, follow-up ob, transvaginal ob, nuchal translucency, ob bio-physical profile, ob bio-physical profile-twins, umbilical artery Doppler, pelvic, trans-vaginal/intra-vaginal, follow up pelvis/limited bladder only, non-vascular extremity (complete), non-vascular extremity (limited), infant hips, vascular access/PICC line, guidance for needle biopsy or amniocentesis.� Follow-up study, carotid, ABI or segmental pressure (unilateral), segmental pressure, lower extremity arterial duplex (bilateral), lower extremity arterial duplex (unilateral), upper extremity arterial duplex (bilateral), upper extremity arterial duplex (unilateral), lower or upper venous (bilateral), lower or upper extremity venous (unilateral), abdominal Doppler (complete), abdominal Doppler (limited), aorta/iliac duplex, bypass graft, hemodialysis access, scrotum/testicular, extremities, miscellaneous/other, venous Doppler, knee, non-vascular extremity, aorta/AAA, neonatal head, thyroid/neck/parotid/parathyroid, chest. The Service Unit will provide a �chaperone� if requested in the case of trans-vaginal/intra-vaginal ultrasounds. The Ultrasonographer will provide quality assurance monitors for images taken and will maintain a separate log for the Contractors� billing purposes. The Contractor shall provide services in such a way as to comply with TJC standards. The Contractor shall supply all tech-sheet paperwork. The Contractor shall provide all SDS on probe disinfectants, gels etc. The Contractor shall provide the service unit with yearly competency documentation on technologist(s) to include GUS/transvaginal probe disinfection. The Contractor shall supply the service unit with all service and annual preventative maintenance records on ultrasound equipment. The Contractor shall provide services on a fee-for-service basis, using a fee schedule that has been agreed to by the Service Unit. The Contractor shall possess liability insurance for acts of liability performed by their Technologists and/or their equipment. The Contractor shall have a history of providing these imaging services competently and safely to other customers. The Contractor shall provide ultrasound services for a minimum of three (3) half-day ultrasound clinics per week, the days of which are at the discretion of the Service Unit.� The schedule will be determined on a �as needed basis� depending on the demand of exam load.� The Technologist(s) must adhere to all Indian Health Service Rules and Regulations.� All Technologists will be properly credentialed, have passed proper security background examination, provide all immunizations, and complete mandatory facility wide trainings prior to providing the service at the Service Unit.� Individuals providing services under contract, which involves regular contact with Indian children, are subject to a character investigation in accordance with the �Indian Child Protection and Family Violence Prevention Act� P.L. 101-630.� The Indian Health Service Personnel Office will conduct these investigations following an award of a contract unless judgment of the Contracting Officer, in consultation with the Personnel Officer, investigation may be waived based on prior investigations on file.� Until a character investigation has been completed and the Contracting Officer notified, such individuals shall not have unsupervised contact with Indian children.� The Contracting Officer has the authority to waive the Character Investigation of a contractor.� The �Agreement to a Temporary Waiver of Character Investigation� authorizes an agency to hire a contractor prior to the completion of a background check, provided that at all times the contractor is within sight and under the supervision of a government employee who has successfully completed the National Agency Check with written inq
