- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) Unique Entity ID (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The Greater Los Angeles VA Healthcare System located at 11301 Wilshire Blvd., Los Angeles, CA 90073 is seeking a potential qualified vendor for the proposed Draft Scope of Work described in the following. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 621511 ($36.5 Million Dollars). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by e-mail to edward.condreay@va.gov. All information submissions to be marked Attn: Edward Condreay, Contract Specialist and should be received no later than 4:00 pm PST on October 11, 2022. Overview 1.1 The Contractor shall provide all necessary and appropriate labor, certified technicians, travel, supervision, materials, parts, equipment, tools, control and disposal of hazardous/nonhazardous licensed material, training, proof of licensing, documentation of required services, and permits when required to provide Media Fill Testing (MFT) & Gloved Fingertip Testing (GFT) for evaluation of personnel performance for employees in the inpatient pharmacy department assigned to compound sterile products at the Department of Veterans Affairs located at 11301 Wilshire Blvd, Los Angeles, CA 90073. MFT and GFT is required on an on-going basis to ensure that compounding personnel are competent in proper hand hygiene and garbing procedures and are utilizing aseptic work practices. This testing is mandated by the United States Pharmacopeia (USP) 797 standards. Scope of Work Contractor shall provide the appropriate number of MFT and GFT test kits and evaluation of personnel performance for employees assigned to compound sterile products, number of which may equal 28 individuals. Contractor shall conduct all testing procedures according to USP standards including, but not limited to, USP Pharmaceutical Compounding Sterile Preparations and USP Microbiological Control and Monitoring of Aseptic Processing Environments. Specific Tasks Contractor shall provide all necessary materials, labor, supervision, and management to coordinate MFT and GFT for VA Greater Los Angeles Healthcare System. Contractor shall provide detailed information on the GFT and MFT process to be conducted when requested. Contractor shall conduct initial MFT and GFT testing for employees newly assigned to the compound sterile preparations, then renewal testing every 6 months there after for a period not to exceed one year. Contractor shall provide detailed documentation of positive or negative results to service within 21 days of testing completion. Contractor shall coordinate testing dates with VA Greater Los Angeles Healthcare System Inpatient Pharmacy Management team as needed, with at least a 48-hour notice. Contractor shall provide retesting in the event of inconclusive or positive results. Contractor Qualifications and Requirements 4.1. Contractor shall have trained and accredited personnel conduct MFT and GFT in accordance with standards outlined by USP Chapter . Monitoring Procedures 5.1. The VA staff shall be responsible for monitoring the Contractor's performance. Performance deficiencies or poor performance identified during the term of the contract shall be reported to the Contractor. Upon notification of deficiencies or poor performance, the Contractor shall be given 10 days to take corrective action. If determined appropriate, the Contracting Officer and/or other VA staff members shall meet with representatives of the Contractor to review and resolve issues. Changes/ Authorized Services 6.1. Only those services specified herein are authorized under this contract. Before performing any service or repairs of a non-contract nature, the Contracting Officer must be advised of the reasons for this additional work. If appropriate, the Contracting Officer may authorize the additional services or repairs under a separate purchase authorization. The Contractor is cautioned that only the Contracting Officer may authorize additional services or repairs and that reimbursement will not be made unless this prior authorization is obtained. Contracting Officer 7.1. No service shall be provided without the approval of the Contracting Officer of this contract. The Contractor shall not accept any instructions issued by any other person(s) other than the Contracting Officer or his/her delegated representative acting within the limits of his/her authority VA Policy and Regulations Badging: All Contractor personnel are required to wear I.D. badges issued by the VA Greater Los Angeles Healthcare Systems security office during the entire time he/she is on Government property. I.D. badges must have an identification picture and shall state the name of the individual and the company he/she represents. Parking: It is the responsibility of the Contractor s personnel to park in the appropriate designated parking areas. The VA will not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstance Smoking Policy: Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas Weapons: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court). Operational Hours Contractor shall provide testing during normal operating hours. Operating hours are Monday thru Sunday 0000 to 2400 hrs. The Contractor is not required to provide services on the following U.S. Government holidays, nor will the Contractor be paid for these holidays except for emergency/rush requests and will be scheduled between the contractor and the POC only. National holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Juneteenth National Independence Day 19 June Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
