- THIS REQUEST FOR INFORMATION (RFI) 36C26222Q1683 - IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339113 (size standard is 750 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. Vendor Requirements: Period of Performance is 180 Days After Receipt of Order (ARO), this time frame includes the installation. Vendor shall quote all items under this requirement. Vendor shall meet the Salient Characteristics in the Statement of Work (SOW). Vendor shall respond questions 1 through 8 of the Request of Information (RFI), see below. STATEMENT OF WORK FOR BARRIER FREE PRIVACY CURTAIN SYSTEM / DISPOSABLE TEXTURED CURTAINS FOR NORTHERN ARIZONA VA HEALTHCARE SYSTEM COMMUNITY LIVING CENTER (CLC) IN BUILDING 148 Introduction The purpose of this statement of work (SOW) is to outline the tasks and responsibilities of the successful offeror in relation to providing new barrier free track and privacy curtain track system and textured curtains (one for the doorway and another located between two patient bed to ensure patient privacy) - Lorton Group Patient Lift Privacy Systems (Non-Ceiling Mounted Privacy System) oval track or Equal to, compatible Easy-On Off textured disposable curtains or Equal to, to be used at Northern Arizona VA Health Care System (NAVAHCS) installed in fifty-five (55) rooms. Background The Northern Arizona VA Health Care System (NAVAHCS), Prescott, AZ, VAMC has a need for disposable patient privacy curtains. This need arose from the installation of new patient lift systems installed 55 patient rooms in the Community Living Center (CLC) in Building 148. The lift installation required the removal of existing patient privacy tracks and curtains. This created the current situation of needing to replace those curtains. Patient Privacy curtains are mandated by the Joint Commission. The curtains need to provide privacy 1) to the patient from visibility of other patients in their room and 2) from visibility from the room door that opens into the wing hallway. The patient rooms are not identical. A pre-award site visit is highly recommended to understand the full scope of the interaction between the patient lift systems moving parts and the curtain placement. The curtain system will be designed so that the use of the privacy curtains will not interfere with the operation of the patient lift system which is an x-y ceiling mounted system with an active or mobile rail. The privacy curtains need to be installed in a manner that does not interfere with doors, wall mounted patient televisions, or medical equipment such as medical gas use. Drawings of curtain system designs will be provided to the VA for review and approval prior to the start of installation. Pictures of patient room with ceiling mounted patient lift system (pictures provided below for guidance purposes only): Scope This procurement is to provide Northern Arizona VA Healthcare System with fifty-five (55) configurations of Lorton Group Patient Lift Privacy Systems oval track or Equal to with Easy-On Off textured cubical curtains or Equal in exam and procedure rooms. Field verification and installation are required on-site. General Requirements Vendor shall provide to VA Northern Arizona Healthcare System, fifty-five (55) configurations of Lorton Group Patient Lift Privacy Systems track or Equal to with two sets (110) total of Easy-On Off textured cubical curtains or Equal to at Northern Arizona VA Healthcare System. Mandatory Tasks The vendor to coordinate with the VA point of contact (POC) for the delivery of items. Notification of expected shipment date shall be provided no later than (14) business days prior to shipment. The vendor shall verify quantity, configuration. Vendor to make a site visit to field verify upon award prior to product installation. The Vendor shall ship Lorton Group Patient Lift Privacy Systems oval track or Equal to, Easy-On Off textured cubical curtains or Equal to to the 500 Hwy 89N, ATTENTION: Warehouse, Prescott, AZ 86313. The vendor shall also install to COMMUNITY LIVING CENTER (CLC) IN BUILDING 148, 500 Hwy 89N, Prescott, AZ 86313. Once product is received at the site, the vendor is to be responsible for any replacement punch items to be delivered within (10) business days to ensure product to be installed complete. Vendor shall provide all necessary materials, equipment, labor, supervision, and management to provide, deliver, and install items. The vendor s installers shall deliver the Lorton Group Patient Lift Privacy Systems oval track or Equal to with Easy-On Off textured cubical curtains or Equal to, assemble and install onsite. The vendor shall contact the VA POC to coordinate the punch walk for final inspection of product installation. Period of Performance/Acceptance The Period of Performance (PoP) is 180 Days After Receipt of Order (ARO) this time frame includes delivering, assembling and installation of the equipment. Delivery Schedule Coordination with the VA POC is required for the delivery of items. Notification of expected shipment date shall be provided no later than (14) business days prior to shipment. The contractor shall provide a delivery schedule based on the procurement requirement (14) days after award of the contract. Schedule and Order Management The above delivery schedule is based upon the current construction schedule for this building where Lorton Group Patient Lift Privacy Systems oval tracks or Equal to with Easy-On Off textured cubical curtains or Equal to will be installed. The vendor shall not increase prices for a change in delivery date for up to 90 days due to construction delays. The awardee actual delivery date will be confirmed by VA POC upon award Post- award the vendor will coordinate delivery prior to beginning any work. Once the dates have been confirmed, modifications to the schedule are subject to written approval by the VA POC and the vendor. Schedule changes that are beyond the dates specified in the contract will require approval by the Contracting Officer and a contract modification. Vendor shall follow FAR 52.212-4 (f) for Time Extension Requests. Delivery and Receiving Delivery and receipt of the proposed items is anticipated to be directed to the location identified below. Confirmation of delivery location will be provided post-award. To coordinate delivery, vendor shall contact the VA POC. The delivery of items identified in this document shall take place during normal business hours which are defined as: 0700 to 1530 (i.e.: 7:00am to 3:30pm Mountain Standard Time), Monday through Friday, and excluding Federal Holidays. Trucks shall be unloaded, moved from the alley, and then brought back to the alley if required to haul out any waste, tools, or excess materials. Vendor shall coordinate with the VA POC details of the designated area for the delivery/deliveries. Labeling of delivered items shall include the VA facilities contract number and VA purchase order and contract number for identification and reference upon receipt of product. One-Year Warranty Easy On Easy Off Disposable Curtains: The Easy On - Easy Off Curtains are open C-type fast install and removal curtain system. curtains to be manufactured flat and sized approximately 10% wider than the track length, unless specified otherwise. The curtain height will be determined by the floor-to-ceiling height and curtains should be finished approximately 10 -12 above finished floor, as suggested by Joint Commission's patient privacy standards. This distance will allow for ease in periodic floor cleaning and prevents mop and/or floor cleaner from coming in direct contact with cubicle curtain, soiling the treatment. The C-type rings to be inserted and spaced minimum 6 and maximum 8 apart on a minimum 4 and maximum 6 National Fire Protection Association (NFPA) approved vinyl breathable mesh fabric that is integrated to minimum ½ and maximum 1 vertical and horizontal hole size opening mesh (minimum 70 percent open) allows for fire sprinkler penetration in the unfortunate case of fire. The mesh will be required to have a minimum 18 drop from sprinkler head to fabric. If the distance of the Flat Track is farther than the 18 drop then mesh may not be necessary to meet patient privacy standards. The curtains must use a commercial grade of molded synthetic polymer C-type rings that allow for Easy On -Easy Off handling of environmental services and nursing staff. The ring system must mount on the approximately between .25 -.5 x 1.5 -2 Flat Track and must glide effortlessly without binding over the life of the curtain. The bottom hem of the cubicle curtain is double folded and minimum 2 and maximum 3 . The side hems are double folded and minimum 1 and maximum 2 . All hems and seams are sewn with a double needle lock stitch. All vertical seams are surged and sewn with a double needle lock stitch for maximum durability. The fabric will contain the following characteristics to be approved by the facility: meet NFPA 701 flame spread requirements for vertical flammability (flame spread certification paperwork must be supplied), fabric must have a antimicrobial properties that have been proven to reduce bacteria on soft surfaces, must be evidence based third party tested, produces bacterial reduction within one hour and antimicrobial properties must be intrinsic and irreversibly bound to the polymers of the fabric and are able to withstand a minimum of 200 commercial launderings at a temperature of not more than 160 degrees Fahrenheit (71 degrees Celsius). Vendor Responsibilities The vendor shall be responsible for delivery of all the products listed in Section 11. The vendor shall be responsible for configuring and assembling fifty-five (55) custom configurations of Lorton Group Patient Lift Privacy Systems oval track or Equal to. The vendors shall provide installation for fifty-five (55) custom configurations of Lorton Group Patient Lift Privacy Systems oval track or Equal to, two sets of Easy-On Off textured cubical curtains or Equal to one-hundred ten (110 total). The vendor shall provide design services- verification of quantity, custom configurations, and field verification. The vendor shall not provide refurbished equipment. After award, the vendor shall provide the VA POC shop drawings must be provided as PDF and/or AutoCAD. After award, the vendor shall provide the VA POC the warranty literature information must be provided. Minimum (1) One-Year Warranty. Job Site Cleanliness There are no dumpsters available for vendor use. All vendor trash is to be handled by the vendor and removed from site daily. Assembly, Installation and Final Acceptance The vendor is required to manage and coordinate delivery and acceptance at NORTHERN ARIZONA VA HEALTHCARE SYSTEM with the VA POC. The vendor is required to define the need for a staging area to accommodate item delivery, if needed. Post-award, this information will be confirmed with the vendor. The vendor is required to provide tools, labor and materials to complete assembly and installation of the items detailed in this document. The vendor shall protect all finished spaces and surfaces as required from delivery - damage. The vendor shall use covering and protection to the extent necessary to prevent damage to finished spaces. Any damage occurred during delivery is the responsibility of the vendor. The vendor will be responsible for paying and repairing any damage or noted deficiencies to finished spaces and surfaces that occur as a result of the vendor s (or associated sub-contractors) installation. During the entire duration of assembly and installation, the vendor will have a representative reachable via phone as the vendor s contact, and to serve as the interface between the vendor and VA POC. All instructions provided from the VA POC to the representative will be binding as if given to the vendor s main contact. The VA POC may provide specific instructions, however, only the Contracting Officer may change the terms or conditions of the contract. Design Services The contractor will be required to provide basic design services and layout for curtain tracks being requested to included quantity per each specific item and room placement as part of the design submittal described in this contract. The VA POC will provide drawings for the floor plans to the contractor after award. Contractor shall be responsible for all field measurements. Contractor shall not rely on floor plans for final measurements. The vendor shall provide a designer with a 2 year minimum experience with the specified product under this requirement. Meeting Requirements for the vendor: Vendor shall coordinate conference calls with the VA POC as required to review curtain track layouts, to share progress and to make necessary revisions. Meetings shall be documented by vendor, with meeting minutes provided to VA designated POC for review and concurrence. After award, a site visit to the Northern Arizona VA Healthcare System COMMUNITY LIVING CENTER (CLC) IN BUILDING 148 for field measurements is to be coordinated with the VA POC. All calls, meetings, and site visits will be during normal business hours which are defined as: 0700 to 1530 (i.e.: 7:00am to 3:30pm Mountain Standard Time). Monday through Friday, and excluding Federal Holidays. During first meeting the vendor shall provide the VA POC with the PDF/CAD drawings showing layouts of awarded product. The vendor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to VA POC a remediation plan within 5 business days. Design Deliverable requirements: The vendor shall allow for three (3) revisions per line item included in design services including updating AutoCad Drawings and/or PDFs as request by the VA POC. The vendor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by the VA POC. The product must not be placed into production without clearance from the VA POC. The vendor shall provide final drawings of product layout for sign-off prior to scheduling manufacturing. The vendor shall track manufacturing schedule and notify VA POC with updates of estimated completion date by email. Product Inspection and Warranty The vendor shall verify and inspect all items. Any and all deficiencies (damage/overage/shortage) shall be brought to the attention of the assigned VA POC in writing, preferably via email, immediately upon identification. The Vendor shall store damaged items in the location designated by the VA POC. The Vendor shall maintain a complete file of all documents relating to each discrepancy and copies of all Discrepancy Reports shall be forwarded to the VA POC daily. Vendor shall coordinate with the VA POC for replacement of damaged, defective, or missing items. Vendor shall inspect to ensure that the curtain tracks are free of surface dirt, clean and polished, free of defects, and that the installation is complete and ready for use. The warranty of an item is to start on the date of acceptance of the products/services by the VA POC. The Vendor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the warranty period. The cost of installation and travel of replacement material and parts shall be borne by the Vendor. Receiving Reports The VA Northern Arizona POC will be responsible to provide all the vendors for the clinic activation with receiving reports. Vendor must ensure they follow up with the VA POC if they have any issues with the receiving reports. (Please contact VA Northern Arizona Healthcare System s receiving department).
