- Department of Veterans Affairs Veterans Health Administration (VHA) Sources Sought Notice Obtain a qualified vendor to provide Scrubs Management Services at Manchester VA Medical Center This is a Sources Sought notice and not a request for quotes. This request is solely for the purpose of conducting market research to enhance VHA s understanding of your company s offered services and capabilities. The Government will not pay any costs for responses submitted in response to this Source Sought Notice. This Sources Sought notice provides an opportunity for respondents to submit their capability and availability to provide the requirement described below. Vendors are being invited to submit information relative to their potential to fulfill this requirement, in the form of a capability response that addresses the specific requirement identified in this Sources Sought. The Veterans Health Administration (VHA) is seeking to obtain a qualified vendor to Scrubs Management Services at Manchester VA Medical Center per the general requirements below. The standard shall be of quality; meeting or exceeding those outlined in the general statement of work as described below. Refer to the statement of work section below for the requested requirement description. This Sources Sought is to facilitate the Contracting Officer s review of the market base, for acquisition planning, size determination, and procurement strategy. Scrubs Management Services at Manchester VA Medical Center INTRODUCTION: Manchester VA Medical Center has a requirement for medical scrubs, and the automated scrubs dispenser service. The service shall encompass the supply, installation, and maintenance of automated scrub dispensing machines and return units. All personnel, equipment, tools, materials, and service shall be provided by the contractor. Contractor shall provide, install, and maintain four (4) Automated ScrubEx Dispenser/Receiver unit, all automated dispensing and return units shall adhere to the provided size restraints. PERFORMANCE REQUIREMENTS: The Contractor shall provide, install, and maintain: Four (4) - Automated ScrubEx Dispenser/Receiver units Four Automated ScrubEx Dispenser/Receiver units to fit into the dimensions of 78 H x 52 W x 16.5 D The Contractor shall track weekly use of garments via an electronic monitoring system and provide a report of items serviced upon request. The Contractor shall provide a rental program for scrub pants, shirts and jackets and provide the VA with an auto-loss replacement policy to replace any scrubs that may become damaged or deemed no longer usable. Automated ScrubEx Dispenser/Receiver unit shall be delivered to proper locations, and install, program, stock and insure proper operations of all systems. The dispensing machines units shall be provided by the Contractor as long as the contract is in place. The machines are to be installed and be fully functioning by the contractor. The machines shall be serviced/maintained by the Contractor for any technical difficulties or machine malfunction. The Contractor shall make every effort to resolve the issue by coming on site as needed, as well as maintain open communication with the COR. Automated ScrubEx Dispenser/Receiver unit locations: Dispenser/Receiver #1 shall be housed within Building 1 Operating room Dispenser/Receiver #3 shall be housed within Building 1 Urgent Care Dispenser/Receiver #3 shall be housed within Building 1 Main Locker Men's Dispenser/Receiver #4 shall be housed within Building 1 Main Locker room Women's Records Management Language: The following standard items relate to records generated in executing the contract and shall be included in a typical procurement contract: 1. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. 2. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. 3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. 4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. 5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. 6. The Government Agency owns the rights to all data/records produced as part of this contract. 7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. 8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. 9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. 10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
