- DEPARTMENT OF VETERANS AFFAIRS STATEMENT OF WORK BOILER PLANT CONTINUOUS OPACITY MONITORS INTRODUCTION The VA Medical Center in West Haven and Newington CT has a total of six (6) boilers that require a Continuous Opacity Monitor (COM) audit as it pertains to the requirements of the State of Connecticut DEEP as well as preventative maintenance and repairs for the recording equipment, sensors and other associated equipment used for monitoring the Opacity of the spent fuel gasses. The contract will have a one-year base period with four option years. SCOPE Continuous Opacity Monitors(COM) Audit The contractor will be responsible for doing a Continuous Opacity Monitors (COMs) Audit as it pertains to the requirements of the State of Connecticut DEEP. Boilers #1, #2, and # 3 which are located in Newington, CT and Boilers #1, #2, and #3 which are located in West Haven, CT shall each have an opacity audit performed each quarter. The contractor will meet the requirements for the monitoring of the air quality of spent fuel gases to the environment. The Contractor shall supply all audit jigs and audit filters, meeting HOCFR60 APP.A+F PSI quality control procedures for COMS s. Filter shall be 8%, 19%, and 30% +/- 1%. Repair Work Orders/Emergency Work Orders The Contractor shall provide all parts and labor for repairs to the system and components. The following work order process shall be completed. The contractor shall submit a work order to the Contracting Officers Representative (COR) to include the following: labor hours, parts, pricing, total. The COR will review the work order to verify pricing and labor hours. The Contracting Officer shall review all repair quotes to ensure pricing is fair and reasonable. The Contractor shall provide a copy of the approved work order to the Contracting Officer (CO) and COR. The Contractor shall invoice upon completion of the work order. Reports The Contractor shall provide original copies of all service records and reports on a form approved by or provided by VA. Service reports shall include hours and names of employees and shall include all parts and materials used. Service reports shall be reviewed by and signed by the Facilities Management Service Representative upon completion of work and prior to the service technician s departure from the station. The Contractor shall submit reports to the State of Connecticut and VA as required per State of Connecticut DEEP and EPA requirements. The written recommendations of the equipment manufacturers. The requirements and recommendations of NFPA 85 Boiler and Combustion Systems Hazards Code including applicable appendices. Medical Center, Facility Services Policy, FS-7. VA Directive 2008-62 PERFORMANCE REQUIREMENTS All work shall be completed such that the integrity of the systems is maintained. All work shall be done in accordance with OSHA, EPA regulations, NFPA codes, with particular attention to NFPA 85, VA codes, Federal, State, and local regulations throughout the course of this project. Applicable VA publications and design manuals are available to the successful bidder at www.va.gov/facmgt. All areas will be left in a clean and workmanlike manner. DELIVERABLES Quote Submission All quotes shall include the following: Completed price schedule which is attached to the solicitation. Completed past performance sheet which is attached. OTHER CONSIDERATIONS FOR PROPOSAL: The provisions of all applicable NFPA, OSHA, EPA, and VA regulation, Federal, State, and local laws or regulations shall be followed at all times. Material Safety Data Sheets (SDS s) must be submitted to the Contracting Officer s Technical Representative (COTR) for approval for all hazardous materials before they are brought to the work site. Two weeks prior notice is required for all work that involves shutting down any of the systems. Work requiring a shutdown may be required to be performed outside of normal working hours. End of day cleanup of the work area(s) is mandatory and all trash shall be bagged and properly disposed of offsite. Any oil waste generated from maintenance of VAMC equipment shall be identified and communicated to the GEMS Coordinator for proper collection onsite. Hot Work Permits (for welding, brazing, soldering, torch cutting, etc.) are to be applied for through the COTR. Penetration permits for any wall penetrations shall also be required. Confined Space Permits (for entry into boiler cavities) are to be applied for through the COTR. Contractor vehicles, including those of their employees, are to be parked in the lower level of Lot D. Exclusion: Any vehicle directly involved in the cleaning and removal of debris from the boilers. Smoking permitted in designated locations only.
