Contract Title Service to perform preventive maintenance and battery replacement per quotation QUO-22874-Z8M2G6, for the Toshiba Uninterrupted Power Supply (UPS) Unit, which provides backup power for our Canon AQUILION ONE GENESIS 640 CT System in Radiology. All work relative to this service will be performed at Overton Brooks VAMC, Shreveport LA. 3. Background The Toshiba UPS is a one-of-a-kind 3-phase system that provides backup power for our Canon AQUILION ONE GENESIS 640 CT System in Radiology. The Canon CT system is critical to the imaging services that we provide, used to diagnose cancer, heart disease, as well as other life threatening diseases. It is crucial to the consistent operation of the CT system that it has consistent 3-phase backup power. The Toshiba UPS is due PM and battery replacement, which is required to facilitate its operation in providing efficient 3-phase backup power for the CT system. 4. Specific Tasks-Deliverable Product A certified Toshiba service engineer shall perform preventive maintenance and battery replacement for our Toshiba G9000 100-300KVA UPS, S/N 161100908, located at Overton Brooks VAMC, Shreveport, LA. Per Toshiba Quotation QUO-22874-Z8M2G6. 5. Hours of Operation All tasks for this contract shall be accomplished Monday through Friday, 8:00 am through 5:00 pm, excluding federal holidays. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Overton Brooks VAMC shall not furnish parts and/or test equipment for the performance of this contract. It is the responsibility of the contractor to bring the appropriate equipment; and/or supplies necessary to complete all work, as required. 7. Place of Performance. All work shall be performed at Overton Brooks VAMC. 8. Information Security Requirements Vendor will not have access to sensitive patient information while performing this service.
