- STATEMENT OF WORK PONTIAC CBOC LAB COURIER SCOPE: The successful offeror shall provide all vehicles, gasoline, insurance, personnel, labor and materials necessary to accomplish regularly scheduled same day courier services for and between the John D. Dingell VA Medical Center (JDDVAMC), Detroit, Michigan and the Pontiac Community Based Outpatient Clinic (CBOC). Services shall be provided Monday through Friday, excluding Federal Holidays. SCHEDULE/LOCATION/ITEMS: Work Hours: Pick up at the Pontiac Community Based Outpatient Clinic (CBOC), 44200 Woodward Avenue, 2nd floor, suite 208, Pontiac, Michigan, shall be between 11:00 a.m. and 2 p.m. and delivery shall be made no later than 3:00 p.m. to the John D. Dingell VA Medical Center (JDDVAMC), 4646 John R, Detroit, Michigan, Fourth Floor, Room C4401 Laboratory, Monday through Friday, excluding all Federal Holidays. Offeror must be able to commence work no later than September 17, 2022. Federal Holidays: The eleven (11) holiday observed by the Government are: New Year s Day, President's Day, Martin Luther King Jr. Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Insurance: The Contractor shall have, maintain and show proof of insurance on contractor owned vehicles and professional liability. Inclement Weather Provision: The Contractor shall contact the manager or designee when the Contractor feels that weather indicates adjustment or cancellation of scheduled pick up/deliveries. Contact information will be provided upon award. If the contractor fails to furnish the required services within the time frames established, the Department of Veterans Affairs reserves the right to obtain the service from another source and shall charge the contractor with any excess costs incurred by the Government which resulted from the failure. Contact: The laboratory manager or designee shall be the main point of contact. Contact information will be provided upon award. Description of Items: All items that contain Personally Identifiable Information (PII) shall be transported in a locked container. The contractor is responsible for obtaining the container and providing 2 keys for VA Staff to open and lock the container; one key at the JDDVAMC and the other key to be kept at the drop location in Pontiac CBOC. Items that may be transported include, but are not limited to blood, urine, tissue, and other body fluids. Blood and other body fluid samples shall be transported in a locked container specifically designed for blood transportation. Samples may contain infectious agents. The number of items being transported will be counted prior to being placed into the locked container and recorded on a spread sheet. When the items are delivered they will again be counted, recorded and receipt signed. In accordance with VAAR 801.603.71 (a) (1) & (2), only designated personnel can receive and inspect packages and deliveries. These logs will be submitted to the manager on a monthly basis. QUALIFICATIONS: Offers will be considered only from those offerors who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. Offeror should have at least two years of work history in this capacity. Provide a list of all courier contracts for the last two years (or four most recent contracts) with points of contact for each. Offeror shall submit a brief work plan explaining logistical processes (routes/cargo transfers, etc.). Work plan shall also discuss contingency plans for inclement weather and disabled vehicle/driver. Successful offeror shall meet all requirements of Federal, State or City codes regarding operations of this type of service. Associated staff will maintain current training and certification in accordance with 49 CFR, Parts 171-173. DRIVERS/ATTENDANTS: All drivers shall have attained legal age (the age of 21 years) and shall maintain neat, clean and professional appearance. All drivers shall maintain in their position a driver s license issued by the State of Michigan (or other State), to include any special licensure required for the transporting of infectious materials. Drivers shall not make stops outside the scope of the courier business during the transport of items under this contract. The successful offeror shall sign and engage in a Business Associate Agreement (BAA) with the VA upon award. Drivers shall complete annual VA National Privacy Training and submit proof of completion to manager. Drivers shall have current DOT training for transporting infectious materials. Documentation must be provided to the manager. VEHICLES: Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State of Michigan, to include a Hazardous Materials/Waste Certificate if required. LIMITATIONS: Under no circumstances shall any person other than the driver ride in a vehicle or handle items being transported. Courier shall not transport other than similar medical cargo for other clients while in the possession of VA material. SECURITY: While on VA grounds, contractor shall secure vehicles when left unattended. If vehicle must be left unattended with motor running, a second set of keys will be necessary, so that vehicle can be securely locked. Dingell VAMC 4th Floor, Room C4401 (Pathology and Laboratory Medicine Service) 4646 John R Detroit, MI 48201 VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR ENSURING ADEQUATE COVID-19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS (AMENDED OCTOBER 25, 2021) Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
