Description
(I)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested for preventative maintenance at the Richmond VAMC. A written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ 36C24622Q0752_1 (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05. (IV) Any amendments issued to this combined/synopsis will ONLY be available on the SAM website (SAM.GOV). (V) The Network Contracting Office 6-RPO East intends to award a firm-fixed-price contract for the VAMC RICHMOND VA. (VI) The North American Industrial Classification System (NAICS) code for this procurement is 811219 with a small business size standard of $22 Million employees. This procurement is for all eligible offerors who may submit quotes. All offerors must be registered in the System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. All SDVOSBs must be listed and registered in Vetbiz.gov to be considered for award. (VII) Questions concerning this solicitation shall be addressed to Audrey Wooten Contracting Officer and emailed to Audrey.Wooten@va.gov. All questions or inquiries must be submitted no later than 9 am Eastern Standard time on 07/25/2022. No phone call will be accepted. (VIII) The Government will award a contract resulting from this RFQ to the responsible offeror whose quote MEETS SOW and Items LISTED IN PRICE SCHEDULE. Quotes are due by 10 am Eastern Standard time on 07/27/2022 Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Background. This scope is used in Respiratory and ENT clinics. 3. Scope. This contract is for the Full Factory Refurbishment of our Olympus Ultrasound EBUS Bronchoscope Mdl: BF-UC180F sn: 7822732 asset# 652-EE6051635 This contract is for the Full Factory Refurbishment of our Olympus WA50052A sn: 614238 & WA50050A sn: 628621 and OTV-S7ProH sn: 7746435 4. Specific Tasks. Items that need to be repaired: The endoscope is completely disassembled and inspected; The insertion tube is replaced with other parts replaced as needed. The original endoscope exterior Control Body, Grip, Light Guide Connector, and/ or Eyepiece are retained and are tested to ensure they meet functional specifications. The endoscope is tested to ensure watertight integrity. All critical components are to be replaced, including the lens, and outer tube. In some cases, the surgical instrument's original components may be retained if they meet all functional specifications. 5. Security Requirements No security clause is required. 1. Offeror shall provide an itemized list of the items The offeror shall provide a quote addressing CLINS (1) NO remanufactures or gray market items will be accepted. 52.212-2 Evaluation-Commercial Items (Nov 2021) Basis for Award. The Government will issue a purchase order to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, Price and Other Factors considered. The following factors shall be used to evaluate quotations: (1) Technical Features: meets and /or exceed requirement in the SOW (2) Past Performance: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements (3) Price: The Government will evaluate the price by adding the total of all line item prices. The Total Evaluated Price will be that sum. Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides an additional benefit(s). Quotations may exceed the minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation.
