Description

The United States Military Academy at West Point - NY has a requirement for the procurement of preventative maintenance service for a Thermo Fisher Scientific ISQ Mass El Large Turbo Gas Chromatography Mass Spectrometer System (GC/MS) as listed in the attached Solicitation# W911SD22R0153.� The Federal Government now requires all vendors to complete (fill in) the Solicitation (SF1449) with all quoted offers.� Quotes will not be accepted unless I receive a completed and signed / dated SF1449 to go with your own company quote (no exceptions).� The SF1449 must be fully completed and returned to me as follows... PAGE 1:� Block 12, 17a, and *24 (*Combined Overall Total for the entire Contract)���� PAGE 1:� Block 30 a / b / c� (***must be SIGNED/DATED; can't accept quote w/o signature***)��� PAGE 31: �Unit Price -and- Overall Total � You may submit your own company quote with the SF1449.� However, completion and submittal of the SF1449 is mandatory in order for your quote to be considered (no exceptions). NOTE 1:� This solicitation is unrestricted under NAICS Code 81121; business size $8M.� NOTE 2:� Promotional / Discount Pricing will only be accepted if the offer is available for a minimum of at least 10 business days from the date the solicitation closes (not from the date you submit your quote).� Any offers below the 10-day minimum requirement will have to be offered as Non-Promotional / Non-Discount price only. NOTE 3:� Quotes will only be accepted if they are completed and processed as stated in NOTE 1, 2, 3.� �Please make sure to provide the following data on your quote - or - on your email reply: 1.� Full Company Name and Address 2.� SAMs UEI (Unique Entity Identifier) Number 3.� CAGE Code 4.� TAX ID# (aka �EIN� or �TIN�) 5.� Business Size (""Small""� must be stated on your reply) 6.� The NAICS Code(s) your company uses to identify its business size. 7.� Any applicable Small Business Certification(s) (ie� 8a, WOSB, EDWOSB, HUBZone, SDVOSB, etc.) 8.� Company POC, Email, Phone#.
