STATEMENT OF WORK LABORATORY REFERENCE TESTING SERVICES FOR THE JOHN D. DINGELL VA MEDICAL CENTER DEFINITIONS: JDDVAMC: John D. Dingell VA Medical Center PALMS: Pathology and Laboratory Medicine Service (11LAB) STAT: An urgent request where action (pickup) is required immediately upon notification to testing lab, but no later than 30 minutes from said notification. STAT test results shall be available within 2 hours of specimen pickup. STAT includes blood, body fluid, and fresh tissue samples for testing. Routine: A regular course of testing, not urgent or STAT. Action (pickup) is required within 1 hour of said notification. Routine test results shall be available within 4 hours of specimen pickup. DEFINITION OF NEED: The Contractor shall provide reference laboratory testing to include pick up and transportation of specimens for the patients of the JDDVAMC PALMS and delivery to the testing facility as outlined in the following sections of the Statement of Work: The test menu provided to the JDDVAMC will be varied and based on the patient test orders placed by clinicians. 1) These laboratory tests are not performed in-house for various reasons such as low testing volume or specialized equipment requirements that are not available at the JDDVAMC. 2) Some of the tests shall be sent as routine and depending on the status of the patient, and some shall be ordered as STAT. 3) This contract shall include Consultant Services when deemed necessary by the PALMS staff. DUTIES OF CONTRACTOR: STAT SPECIMENS: The Contractor shall provide courier service to pick-up at the JDDVAMC, Room C4401, PALMS, and collect packaged laboratory specimens that are ready for transport. The courier shall transport the sample to the testing facility 24 hours a day, 7 days a week, 365 days a year (24/7/365) to include all Federal holidays. Upon completion of testing, the laboratory results shall be called into the laboratory via telephone at 313-576-1000 ext. 65066. In addition, the laboratory shall fax test results to 313-576-1120. The contractor shall provide electronic and paper test menu. STAT testing results shall be available within two (2) hours of pickup. Fresh tissue samples for work-up shall be picked up within 30 minutes. ROUTINE SPECIMENS: The Contractor shall provide courier service to pick-up at the JDDVAMC, Room C4401, PALMS, and collect packaged laboratory specimens that are ready for transport. The courier shall transport the sample to the testing facility 24 hours a day, 7 days a week (24/7) excluding Federal Holidays. Upon completion of testing, the laboratory results shall be called into the laboratory via telephone at 313-576-1000 ext. 65066. In addition, the laboratory shall fax test results to 313-576-1120. Routine results shall be available within four (4) hours of pick-up. JDDVAMC PALMS staff shall inform courier what the status of the samples are, either STAT or Routine upon pickup. PLACE OF PERFORMANCE/PICKUP LOCATION: John D. Dingell VA Medical Center (JDDVAMC) Pathology and Laboratory Medicine Services (PALMS) Room C4401 4646 John R St. Detroit, MI 48201 FEDERAL HOLIDAYS: The eleven holidays observed by the Federal Government are New Year s Day, Dr. Martin Luther King Jr. Day, President's Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. CONFORMANCE STANDARDS: The contractor shall maintain full compliance with applicable Federal, State and Local regulations as well as shall maintain a Clinical Laboratory Improvement Amendments (CLIA) certificate of accreditation; The Joint Commission and/or the College of American Pathology (CAP) accreditation. A copy of the CLIA and CAP certificate shall be provided as a part of the submission. PAYMENT: Payment for services satisfactorily performed shall be made quarterly in arrears upon receipt of a proper invoice to the Tungsten Network (https://authentication.tungsten-network.com/login) and Government acceptance only. Payment shall be made by certified invoice and money shall be distributed from the appropriate Detroit VAMC purchase orders. THE JOINT COMMISSION COMPLIANCE STANDARDS: GENERAL: All applicable VA and VHA policies, procedures and directives shall be followed by the contractor. The contractor shall continuously meet or exceed all Joint Commission Hospital standards. The patient population treated at the VAMC Detroit, Michigan, consists of veterans with varying physical, psychiatric, and psychological needs. Care delivered will reflect the VA Medical Centers mission, vision, and values. PATIENT RIGHTS: Protection of all patients' rights is of highest priority. Patient privacy and confidentiality shall be maintained at all times. Computer access shall be granted on a need-to-know basis, and security of computerized information shall be maintained at all times. No patients shall be subject to any research activity without his fully informed consent, subject to study approval by the Investigational Research Board (IRB). The contractor shall abide by the VAMC Code of Ethics, which addresses marketing, admission/transfer/referral/discharge processes, and billing issues. PATIENT SAFETY: The VAMC shall inform the contractor of all applicable Sentinel Event or other Patient Safety Alerts received by the VAMC. For every applicable alert, the contractor shall take the necessary steps to redesign processes to prevent occurrence and provide written feedback to the COR describing prevention actions taken. All events related to patient injury, medication errors, and other breeches of patient safety shall be documented utilizing the contractor's format and reported to the COR utilizing the VAMC Incident Report. The VAMC shall provide the contractor with VAMC Sentinel Event definitions in the start-up phase. The contractor shall verbally notify the VAMC COR immediately when a Sentinel Event has occurred, which is to be followed by written notification within one (1) business day or sooner if requested by the medical center. The VAMC shall determine when a Root Cause Analysis (RCA) is required of the contractor. The contractor shall conduct all Root Cause Analysis according to VAMC policy and within established time frames (45 days) Completed RCAs shall be provided to the VAMC COR. PERFORMANCE IMPROVEMENT: The contractor shall comply with the VAMC Performance Improvement (PI) program as applicable to the Pathology and Laboratory Medicine Service. Pathology and Laboratory Medicine Service shall monitor the vendor's Test Assay Turn Around Times (TATs) on a quarterly basis. Vendor shall provide a quarterly TAT report for all patient testing requests sent for analysis. Quality Assurance Technologist shall provide a quarterly report to Detroit management team for review Contractor PI activities may be directed by the VAMC, VISN or VHA, as well as accreditation or licensing bodies. The contractor shall abstract, compile and report P1 data as requested by the VAMC collection methodology, report format, and reporting deadlines shall be defined by the VAMC. Examples of required PI activities include the VHA Performance Measures/Clinical Practice Guidelines program, and the Joint Commission ORYX or Core Measurement program. It is noted that PI activities are dynamic and subject to change, as driven by cm-rent priorities and resources. In all cases, the contractor shall continually assess for, and act upon, clinical and administrative improvement opportunities. High priority shall be given by the contractor to the prevention of risk for the patient. The contractor shall implement process-related internal controls to prevent risk to the veteran. VA monitoring for continuous compliance with Joint Commission standards shall be directed by VAMC staff and communicated to the contractor through the COR. VA aggregate and/or practitioner-specific data collected by the VAMC shall be provided to the contractor in accordance with established reporting schedules. VA measures related to contract compliance shall be identified by the COR. VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR ENSURING ADEQUATE COVID-19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS (AMENDED OCTOBER 25, 2021) Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
