- Black Hills Legionella Testing Sampling and Dental Waterline Sampling Contract Attachment A Example of Requirements Item Information Contract YEAR: 1 September 2022 31 August 2023 ITEM NO DESCRIPTION EST Annual QTY UNIT 0001 Quarterly collection of water samples and analysis of water samples to test for Legionella at Fort Meade Medical Center and associated facilities. Includes all necessary sampling equipment, collection of temperature readings, pH level readings, and residual biocide level readings for each sample, labor to collect samples, proper preparation and transportation of samples, analysis, report of analysis, and technical assistance on sample result interpretation. Associated facilities include the Rapid City TR House and the Sturgis TR House. 352 Samples collected with labor to collect samples and samples analyzed 0002 Follow-up analysis of water samples to test for Legionella at Fort Meade Medical Center and associated facilities as a result of any positive findings of Legionella or as requested at the discretion of the COR and/or Site POC up to and including 10% of total samples collected at each health care system annually. Associated facilities include the Rapid City TR House and the Sturgis TR House. 35 Sample containers, shipping containers, and analysis 0003 Additional Sampling Collection of water samples beyond the 10% non-routine for positive findings of Legionella or for other reasons requested by the COR and/or Site POC at any site covered under the contract. This sampling will be paid for separately from the remainder of the contract, with the price set per sampling location and per samples required. As required and requested Sample collection and samples analyzed 0004 Supply of water sampling containers, information on proper collection of samples for total colony forming units of heterotrophic water bacteria per milliliter of water count, and analysis of water samples for the Fort Meade and Hot Springs Medical Center Dental Clinics to ensure compliance. 180 Sample containers, shipping containers, and analysis 0005 Follow-up supply of Dental water sampling containers and analysis for Fort Meade Medical Center Dental Clinic for sampling as a result of a report of over 200 colony forming units of heterotrophic water bacteria per milliliter of water count or at the discretion of the facility. Samples requested up to and including 10% of total samples collected at each health care system annually. 18 Sample containers, shipping containers, and analysis 0006 Additional supply of Dental water sampling containers beyond the 10% non-routine for sampling as a result of a report of over 200 colony forming units of heterotrophic water bacteria per milliliter of water count or for other reasons requested by the COR and/or Site POC for any site. This sampling will be paid for separately from the remainder of the contract per sample required. As required and requested Sample containers, shipping containers, and analysis 0007 Quarterly collection of water samples and analysis of water samples to test for Legionella at Hot Springs Medical Center and the Pine Ridge TR House. Includes all necessary sampling equipment, collection of temperature readings, pH level readings, and residual biocide level readings for each sample, labor to collect samples, proper preparation and transportation of samples, analysis, report of analysis, and technical assistance on sample result interpretation. 560 Samples collected with labor to collect samples and samples analyzed 0008 Follow-up analysis of water samples to test for Legionella at Hot Springs Medical Center and the Pine Ridge TR House as a result of any positive findings of Legionella or as requested at the discretion of the COR and/or Site POC up to and including 10% of total samples collected at each health care system annually. 56 Sample containers, shipping containers, and analysis
