SOURCES SOUGHT NOTICE RFI-2022-PRH-005 THIS IS NOT A REQUEST FOR PROPOSAL (RFP) OR REQUEST FOR QUOTATION (RFQ) This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The purpose of this procurement is to solicit for locate eligible and qualified firms both large and small business able to provide University Fellowship Medical Doctors for the Pine Ridge Indian Health Hospital, Pine Ridge, South Dakota.. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621111 and sized standard in millions of dollars is $10.0. NOTE: Respondent claiming Small Business shall check their social economic status or check Large Business. Anticipated period of performance: To be determined in fourth coming solicitation. Capability statement /information sought: Response to this announcement shall not to exceed 5 pages and should include the following information: 1. A tailored capability statement indicating the firm's ability to provide the related services 2. The respondents' Unique Identity number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc.,) pursuant to the applicable NAICS code and government contract number if applicable. 3. Information should be provided electronically in a Microsoft Word or Adobe PDF format.
