Period of Performance: Base year: 07/29/2022 to 07/28/2023 Option year One (1): 07/29/2023 to 07/27/2024 Option year Two (2): 07/31/2024 to 07/27/2025 Option year Three (3): 07/31/2025 to 07/27/2026 Option year Four (4): 07/31/2026 to 07/27/2027 3. Contract shall be responsible for the maintenance and repairs of all equipment listed. Equipment shall be maintained in good working order. Maintenance services is to include periodic inspections; routine, weekly, monthly scheduled repairs and replacements of parts as needed; and emergency repairs made whenever any of the equipment breaks down unexpectedly. Specific responsibilities of the contractor include: 1. Once a Week: a. Provide the maintenance necessary to obtain a high quality of production and the most useful service life of the equipment. b. Keep detailed maintenance records for all equipment. This includes but is not limited to: Recording the date and the details of the completed service, Noting any deficiencies discovered, Documenting the results of the inspection, Documenting any action(s) performed, and Maintain a data file of certification documents c. All equipment listed shall be inspected and repaired on a regular basis, as prescribed in the owner's maintenance manual published by the manufacturer of each piece of Equipment. Maintain the property according to current technical and manufactures recommended standards. Refer to service procedures, processes, or manuals for the required service specifications and details of items to be serviced. d. Scheduling and completing periodic maintenance according to industry standards. This includes: Lubricating Cleaning Replacing belts Calibration New replacement parts ONLY; NO remanufactured or ""gray market"" items Items offered must meet or exceed the salient physical, functional, or performance characteristics associated with industry standards e. Coordinating and arranging for pick-up and delivery service to be performed on and off the Durham VAMC campus. f. Perform any unscheduled maintenance in an expeditious manner. When unscheduled calibration, maintenance, or repair service is performed, document the service. g. Establish and perform periodic reviews of the calibration/preventive maintenance system. 4. Contractor shall respond to any of EMS requests for emergency repairs within 24 hours of receiving such a request from COR or the EMS representative via phone, fax, email, or other communication method to which the parties may agree. 5. Contractor shall ensure that all services provided under the terms of this contract are performed by persons trained and certified as proficient in the provision of the services provided by an accredited school or manufacturer's training program.
