- 36C250Q003 PRE-SOLICIATION NOTICE THIS IS A PRE-SOLICIATION NOTICE The Department of Veterans Affairs, Network Contracting Office 10, 3140 Governor s Place Blvd., Kettering, OH 45409, will be soliciting a Request for Quotes on or about 07/05/2022 for Perfusion Services at the Department of Veterans Affairs, Richard L. Rodebush VA Medical Center (VAMC), 1481 W. 10th St., Indianapolis, IN 46202. SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) SMALL BUSINESS SIZE STANDARD: $11 Million SERVICES OVERVIEW: The Contractor shall provide contract personnel (s) who are competent, qualified per this performance work statement and adequately trained to perform assigned duties. A Primary Perfusionist shall be available for each case requiring perfusions services. For those cases requiring cardio pulmonary bypass, two (2) Perfusionists shall be available on site. As assigned by the contractor, Perfusionist(s) shall be available to the VAMC for perfusion-related procedures such as open-heart surgical procedures, procedures requiring auto-transfusion, and intra-aortic balloon device troubleshooting. Perfusionist(s) shall also be available on an as-needed and/or emergency basis (within a one-hour recall) during off-work hours. The assigned Perfusionist(s) shall be well versed in all current technologies related to perfusion services such as percutaneous cannulation for minimally invasive and robotic surgery, and implanting and managing ventricular assist devices (VAD) and angio vac procedures, as well as vascular, urology and any other specialty that may require perfusion services. He/she shall work closely with the surgeons and anesthesiologists with respect to management of complex Congestive Heart Failure (CHF) cases and redo cardiac surgeries, as well as aortic surgery. Perfusionist(s) shall be familiar with the operation of all perfusion related equipment so that minor repairs may be made on an emergent basis in the event of equipment failure during a procedure. The perfusion services shall include the following: Set up and operate cardiopulmonary bypass equipment, conduct cardiopulmonary bypass, monitor and document all patient perfusion parameters, and make necessary adjustments for routine and emergent procedures requiring cardiopulmonary support. Operate all associated perfusion equipment including cardioplegia, autotransfusion, cell saver, intra-aortic balloon pump (IABP), ventricular assist device (VAD) and femoral-femoral cardiopulmonary support (emergency bypass) as necessary. Administer medications and blood components via bypass as necessary after consultation with the attending cardiac surgeon or anesthesiologists. Assist in trouble-shooting IABP problems. Provide comprehensive documentation of perfusion related services in the patient s medical record. Clean all perfusion equipment at the end of the procedure, conforming to all relevant hospital policies. Maintain inventory levels and request supply replacements through established VA processes. Promote patient safety before, during, and after the operation. Maintain aseptic technique. Assist in the evaluation of new products and procedures. Infrequent Perfusion duties that may be required at the specific request of the attending cardiac or surgeon include: Ventricular Assist Device (VAD) and Extracorporeal Membrane Oxygenation (ECMO). These long-term life supports, usually 3-14 days in length, require skilled perfusion coverage 24 hours a day (four shifts of six hours each). The specific equipment and supplies for ECMO will be maintained at the VA. Equipment and Perfusionist(s) shall be in the OR and ready to treat patients at least 60 minutes prior to scheduled procedures start time. The Contractor shall ensure perfusionist maintain credentialing requirements and notify the RLRVAMC within 24 hours of Contractor s discovery of personnel not in compliance. The Contractor shall be required to report to the Contracting Officer within 24 hours of Contractor s discovery any disciplinary action taken by the State Board or the institution against providers performing under the contract. The Contractor shall be responsible to provide complete and timely documentation to support the Credentialing Process which will initially commence on award of the contract. For services performed at the RLRVAMC, monitoring of Contractor employee(s) time will be evaluated through sign-in/ sign-out sheets (hard copy or electronic). Contract personnel shall be responsible for signing in/out upon arrival/departure when in attendance. Time sheets will be used by the COR to confirm hours/day and services provided against the contractor s invoices. Standards of Care: The Contractor s Perfusionist(s) care shall cover the range of perfusion services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by: VA Standards: VHA Directive 2006-041 Veterans Health Care Service Standards (expired but still in effect pending revision): https://www1.va.gov/vhapublications/ViewPublication.asp?pub_ID=1443 American Board of Cardiovascular Perfusion: http://www.abcp.org. The professional standards of The Joint Commission (TJC): http://www.jointcommission.org/standards_information/standards.aspx The standards of the American Hospital Association (AHA): http://www.hpoe.org/resources?show=100&type=8 and; The requirements contained within the PWS. IMPORTANT NOTICE: Apparent successful quoters must have a current VETS 4212 report on file with the Department of Labor, registered in SAM, and must apply for and receive verification from the Department of Veterans Affairs Center for Veterans Enterprise (CVE) of their SDVOSB status in accordance with 38 CFR Part 74 and VAAR 819.709 by submission of documentation of veteran status, ownership and control sufficient to establish appropriate status, quoters must be both visible and verified by the Department of Veteran Affairs CVE at the time of submission of bid. Failure to be both verified by CVE and visible on https://www.vip.vetbiz.gov at the time of bid submission will result in the quote being deemed non-responsive. All quoters are urged to contact the CVE and submit the aforementioned requirement documents to obtain CVE verification of their SDVOSB status if they have not already done so.
