2 | Page REQUEST FOR INFORMATION This Request of Information (RFI) is to seek potential socio-economic concerns SDVOSBs/VOSBs only who may be able to provide nursing services. This RFI is IAW 38 USC 8127 (C). This RFI does not guarantee any future procurements/awards with the Department of Veterans Affairs. REQUIREMENT: 3 FTE RN S, 6 FTE LPN s and 10 FTE CNA s PLACE OF PERFORMANCE: East Orange VAMC and Lyons VAMC PERIOD OF PERFORMANCE: one (1) base period of six (6) months and one (1) six (6) month option period. ADDRESS: New Jersey VA Medical Center 385 Tremont Avenue East Orange, NJ 07018 Lyons VA Medical Center 151 Knollcroft Road Lyons, NJ 07939 DESCRIPTION OF SERVICES: New Jersey Healthcare system has a need for 3 FTE certified Registered Nurses, 6 FTE Licensed Practical nurses and 10 FTE Certified Nursing Assistants to perform services on-site only at the Department of Veterans Affairs (VA) East Orange and Lyons Medical Centers. Contractors will work 8:00 AM 4:30 PM on a weekday during their initial Orientation. Typically, initial Orientation lasts 2 days. Upon completion of initial Orientation, contractor will continue to be oriented on the assigned unit according to their scheduled shifts. There may be occasional overtime, pre-approved at the discretion of the nurse manager, and the contractor may be scheduled during Federal Holidays. Shifts for each profession is listed below. Contractors staff shall be responsible for signing in and out when in attendance. Time sheets will be used by the Nurse Manager to confirm hours/day and services provided against the contractor s invoices. Long Term Care Registered Nurse (RN) 3.0 FTE: The Long-Term Care RN provides skilled nursing care to long term care patients with chronic illness, or disability and are not capable of caring for themselves for an extended period of time. The RN ensures patient centered, individualized care that promotes independence maximizes patient s quality of life and ensures coordination of services to meet the patient s needs over a period of time. Experienced and qualified Long Term Care Registered Nurses are required to provide additional staffing for the Inpatient Long Term Care Unit at the New Jersey VAMC. All registered nurses shall have at least two years recent experience in long term care. VA experience is preferred. Educational Requirements: Bachelor s Degree in Nursing or a master s degree in Nursing. Contract nurses are preferred to have experience in long term care and accreditation through the American Nurses Credentialing Center (ANCC). A current CPR/BLS certificate by the American Heart Association is required. The shifts for RN positions are rotating. They will be assigned to NJ both EO and LY 3:30PM-Midnight and 11:30-8:00AM shift including Nights, Weekends and Federal Holidays. Licensed Practical Nurse (LPN) 6.0 FTE: Contract LPN will provide skilled nursing to Intermediate/Medical Nursing Home Care Unit and Psychiatric patients. Contract LPN demonstrates increasingly more independence in his/her management of patient care. The Contract LPN demonstrates competence in caring for adult psychiatric, medical and geriatric patients. Under the supervision of the Nurse Manager, LPN works closely with others as needed in Ambulatory Care, Critical Care, Nursing Home units or the Emergency Room. The nurse functions both independently and dependently with guidance and direction from Nurse Manager or designee. LPN shall have knowledge and skills sufficient to prepare, administer, and appropriately document commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient s response to medication administered and the reporting of any noted change in patient s condition to RN or MD/DO. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice. VA experience is preferred. LPN required to be a graduate of a state approved school of practical/vocational nursing. The minimum requirement for all LPN s assigned to the NJHCS shall include Nurses must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. A current CPR/BLS certificate by the American Heart Association is required. The shifts for LPN positions are rotating. They will be assigned to Both NJ EO and LY 3:30PM-Midnight and 11:30-8:00AM shift including Nights, Weekends and Federal Holidays. Certified Nursing Assistant (CNA/NA) 10.0 FTE: CAN will perform requirements with minimal monitoring by a Registered Nurse. NA provides basic, appropriate, effective interventions to individual patients needs according to established facility policies, safety standards and procedures. The contact NA refers to the professional RN when deviations and problems not covered in the assignment occur. The contract NA anticipates the needs of professional staff and works with them as a team member. The Nursing Assistants provide a vital role assisting the licensed nurses providing direct patient care for patients in the Medical Center and residents in the Community Living Center. NA performs a range of personal nursing care which includes feeding, walking, lifting, turning patients and providing related hygiene care. NA performs a range of diagnostic support duties which include taking, recording and reporting to nurses deviations in vital signs and change in patients condition. Assembles necessary equipment. Cleans and cares for equipment in oxygen and suction therapy, frame beds and reports equipment in need of repair. The minimum requirements for all certified nursing assistants shall include at least 1-year prior experience. VA experience is preferred. A current CPR/BLS certificate by the American Heart Association is required. The shifts for CNA positions are rotating. They will be assigned to NJ EO and LY 3:30PM-Midnight and 11:30-8:00AM shift including Nights, Weekends and Federal Holidays. Pricing will be Fixed Price. New Jersey VAMC is accepting candidates who are not located within the geographical area of E Orange and Lyons VAMC. Preferred candidates too be located within the geographical region of Lyons and/or East Orange New Jersey. Projected line items for each healthcare profession hourly rates will be an all-inclusive rate. Subject to agency showing breakdown of pricing.
