- Titlel:� Amendment 0004 1) The purpose of this amendment is to extend the due date for Phase 1 proposals only due to observance of the Federal holiday on June 20, 2022.� � 2) The Phase 1 Proposal due date is now June 21, 2022, by 4:00 PM EST 3) Offerors must acknowledge receipt of this amendment by signing Blocks 15A, 15B, and 15C and return the form when submitting Phase I proposals. 4) Except as amended herein, all other provisions, terms, and conditions remain the same.
