- The combined synopsis/solicitation is being issued as a Request for Quotation (RFQ) with solicitation number FA442722Q0052. All responsible sources meeting all requirements may�submit a quote. The 60th Contracting Squadron (60 CONS) at Travis Air Force Base intends to award a Firm Fixed Price purchase order for the Hemodynamic Monitoring Platform system upgrade. The vendor shall provide all equipment, supplies, materials, and delivery to the 60th Medical Group, David Grant Medical Center, Travis Air Force Base, California as defined in this Statement of Need. The contractor shall perform all requirements to the standards in this contract. David Grant Medical Center (DGMC) requires an upgrade of new technology to three existing Hemodynamic Monitoring Platforms assigned to the Anesthesia Department. Requested upgrade will be for hemodynamic and cerebral tissue oxygenation monitoring capabilities necessary to provide quality care for vascular, thoracic, neuro and major abdominal procedures. Existing platforms are Hemosphere Monitoring Platforms, brand name or equal to. Upgrade package compatible with DGMC�s existing Patient Monitors, Manufactured by Edwards Life Sciences. Upgrade package should include installation and warranty for a minimum of 12 months. The contractor shall provide a quote for all the line items listed below, the extended list of requirements can be found in the Statement of Need, dated 25 March 2022. Failure to quote on all Contract Line Item Numbers (CLINs) will render the quote unacceptable and ineligible for award consideration in the absence of interchanges. Line Item 0001: Hemosphere Clearsight Upgrade Kit (three each), to include: � HEMCSM10 � PC2K � EVHRS � Windows 10 Line Item 0002: Pressure Controller Cover (three each), to include: � PCCVR � HEMAQSR2 kit � HEMCSMUPG kit � EV1000CS Kit � EVNIUPG-US kit Line Item 0003: Hemosphere Foresight Tissue Oximetry Upgrade Kit (three each), to include: � HEMTOM10 � HEMFSM10 � HemoSphere CX Software Update DELIVERY/ACCEPTANCE ADDRESS/ FOB Destination 30 Days After Receipt of Order (ARO): David Grant Medical Center, 101 Bodin Circle, Bldg 777, Travis AFB, CA 94535 See attached documents for all other Terms and conditions for this acquisition.
