Request for Proposal 36C24722Q0684 Page 2 of 2 The purpose of this amendment to RFQ 36C24722Q0684 is to Provide responses to vendor questions Provide a Past Performance Evaluation form The number of Past Performance references requested is a minimum of 3  Question No. Request for Proposal Volume, Section, Page # Question 1 Section E Solicitation Provisions, E.6 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998), Page 43 Clause 52.211-6 Brand Name or Equal; Clause 52.214-21 Descriptive Language The Government has included provisions 52.211-6 and 52.214-21 in the solicitation; however, the Government is not clear in the Statement of Work under Paragraph 4.3 the Brand Name and Model the Government intends to procure. Please provide the Brand Name and Model so Offerors can meet the requirements of these provisions. Answer: We are seeking AvaSure system, brand name or equal to according to the salient characteristics in the SOW. 2 Section E Solicitation Provisions, E.1 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services (Nov 2021; E.7 52.212-2 Evaluation-Commercial Products and Commercial Services (Nov 2021) It is not clear in either provision that the Government intends to award the resultant contract under the Telecare Companion IDIQ Contract and its contract holders. Please confirm that the resultant contract will be a task order awarded under the Telecare Companion IDIQ contract, its terms and conditions, among its contract holders. Answer: This is NOT and IDIQ contract. 3 PWS Section 4.1 Can the Government confirm that the site assessment (including the Site Assessment Report) is an awarded task order activity? Answer: This is not and awarded task order activity as this is not an IDIQ contract. 4 PWS Section 3.1 The period of performance is 12 months. Does the Government require out year option periods in order to procure annual Software License renewals? Answer: Not an IDIQ contract, no option years required.
