- Replace Windows Various Buildings Project 590-19-105 Scope: Provide construction services for the removal and replacement, and all associated calking and sealants as per attached plans and specifications at the Hampton VA Medical Center: General: The following is a brief summary of the Work: REPLACE WINDOWS VARIOUS BUILDINGS BASE BID: Work includes general construction, alterations, removing, demolition and new installation windows construction, alterations, and certain other items as indicated in the contract drawings and specifications sections for select window areas on buildings 110,110A,137A,146 and 114 as required by drawings and specifications at Hampton, VA Veterans Affair Center. ALTERNATE NO.1: All work associated with ITEM I except deduct all work on building 114. ALTERNATE NO. 2: All work associated with ITEM 2 except deduct all work on building146. Place of Performance: Tasks will be performed at the VA Medical Center, Hampton, VA, 23667. Work will be around an occupied healthcare facility in operation 24 hours a day. Specific requirements (see drawings for area of work): DIVISION 01 - General Requirements DIVISION 02 - Existing conditions DIVISION 03 - Concrete DIVISION 04 - Masonry DIVISION 05 - Metals DIVISION 06 - Wood, Plastics and Composites DIVISION 07 - Thermal and Moisture Protection DIVISION 08 Openings DIVISION 09 Finishes DIVISION 12 Furnishings Period of Performance: Contractor shall commence work under this contract promptly after Notice to Proceed (NTP), execute the work diligently, and achieve final completion and acceptance including final cleanup of the premises within seven-hundred thirty (730) calendar days including submittals. Specific site requirements: Contractor and all subcontractors shall obtain a flash badge from the Police Service, provide proof of OSHA 10 training completion, provide TB test results, and shall abstain from smoking within the perimeters of the facility. All individuals that enter any VA facility are required to wear a face mask and will be pre-screened for COVID19; To include Veterans, employees, visitors, volunteers, and contractors. Construction Safety Requirements: - see attached Hampton Construction Safety Handbook and section 01 35 26 Submittals: - see sections 01 00 00, 01 33 23, and all applicable trade sections Quality Control: - see section 01 45 00 Schedules: - see section 01 32 16 .15 Environmental and Waste Management: - see sections 01 57 19, and 01 74 19 VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
