Sources Sought Only This sources sought is issued solely for information and planning purposes � it does not constitute a Request for Proposal (RFP) or a promise to issue a RFP in the future. This sources sought does not commit the Government to contract for any supply or service. The Government is not currently seeking proposals. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The sources sought responses are advised that the Government will not pay for any information or costs incurred in response to this sources sought; all costs associated with responding to this sources sought will be solely at the interested party�s expense. Not responding to this sources sought does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on beta.sam.gov. It is the responsibility of the potential offerors to monitor this site for additional information pertaining to this requirement, if any. Purpose and Objective:� The Centers for Disease Control and Prevention (CDC), The Occupational Health and Safety Office (OHSO) has a multifaceted mission which covers all facilities that have Centers for Disease Control and Prevention (CDC) employees stationed at them. OHSO provides leadership, oversight and service for the overarching CDC Health and Safety Program which proactively ensures safe and healthy workplaces at CDC worksites for CDC employees, contractors, and visitors. OHSO promotes health and safe work practices to prevent injury and illness and services, as well as provides direct support services to employees at all levels within the CDC organization on health, safety and environment-related matters while assuring compliance with all the applicable federal, state, and local health, safety and environmental laws and regulations.The Industrial Hygiene and Safety Branch oversees hazard and job risk assessment and mitigation for CDC and oversees �all �agency� OSHA �required �programs. OHSO is seeking to determine the availability of potential large and small business vendors (e.g. 8 (a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantage business, and women-owned small business) to conduct ergonomic assessments at CDC campuses nationwide. The assessments will be performed in the following locations: Roybal Campus - All buildings Chamblee Campus - All buildings Corporate Square -� Buildings 1, 8, 10, 11, 12 Century Center III � Buildings (partial) � 1825, 2400, 2500 The District at Chamblee- 2877, 2900, 2920, 2939, 2945, 2960 Ft. Collins - All buildings Research Triangle Park - All buildings D.C. area � Hyattsville, Patriots Plaza, metro DC The Contractor shall conduct the Ergonomic Assesssment as follows: Contractor shall receive completed contact information and ergonomic questionnaire from CDC to initiate contact with CDC staff needing assessment. Contractor shall review ergonomic questionnaire and attempt to contact the employee to schedule a site visit by an ergonomist.� Site visits will be scheduled at the convenience of the affected employee between the hours of 8:00am-4:30pm, Monday-Friday. Contractor shall make and/or provide instructions on all adjustments to the chair, keyboard platform, monitor, and phone position to properly align the individual at the workstation.� Contractor shall counsel the individual on proper ergonomic alignment and work practices in office, laboratory, or industrial settings. Contractor shall anticipate at least a half hour for each assessment to be conducted and a half hour to write each report. This time shall not include time it takes for contractor to park, enter campus, security, screening, and set-up. The contractor�s assessment shall include at least the following: Identify and document the presence of observed musculoskeletal disorder (MSD) hazards. Basic MSD hazards include but are not limited to: High forces exerted by workers Awkward postures High repetition Vibration Contact stress Cold temperatures Evaluate whole body postural MSD and ergonomic design risks associated with job tasks using a systemic process (checklist, risk assessment tool, or workstation assessment tool. Evaluate required body posture, forceful exertions, type of movement or action, repetition, and coupling associated with tasks performed.� An assessment should address all body regions: wrists, forearms, elbows, shoulders, neck, trunk, back, legs and knees. 7. After assessment data is collected, and/or scored, the contractor shall document all findings in a comprehensive report that includes the tool used, including any observations/findings/risk scores and mitigation recommendations. Period of Performance: Base Year + 3 Option Periods Responses: Interested parties believing that they possess the expertise and experience to meet the Government's needs described above are invited to submit a capability statement highlighting their experience performing in person ergonomic assessments nationwide in office, laboratories, and industrial settings. Interested parties are requested to include the following information in their response: 1. Company name, address, phone number; 2. IEU number and CAGE Code, as registered in the System for Award Management (SAM) at http://www.betaSam.gov/ 3. Business size and Classification (e.g., 8(a), HUBZone, etc.) pursuant to the applicable NAICS code of 541990.[GS(1]� 4. Small Business Administration Business Opportunity Specialist (name, email address, phone number) if applicable. 5. Point(s) of contact including e-mail addresses and telephone numbers for contacts 6. This is strictly market research, and the Government will not entertain any questions or phone calls.
