Description: Re-Solicitation: *Revision to SOW is page 12 only for Pine Ridge Dental Department � added quantity 1 Kavo Nomad Dental Handheld Unit, and clarified that the GE Proteus at each location are dual detectors *Revisions to SOW page 1 for section 1.5 Locations � added physical location and geographic distance description *A billable line item for the requirement H � for duties and report for the Radiation Safety Officer Audit, which has always been a billable and required report component on the SOW.� This is a Firm Fixed-Price, Non-Personal Service acquisition for Medical Physicist Services for the IHS Pine Ridge Hospital located in Pine Ridge, South Dakota. Contractor is to perform according to the Statement of Work (SOW). The �unit price� is an all-inclusive cost. Period of Performance: Physical survey must be conducted during May of each year and reports are due within 30 days of completion.� Assistance to create corrective action plan, review plan with program director, and re-evaluate to ensure compliance is acceptable may occur within the following 11 months.� Total Period of performance is a 1-year contract.
