- STATEMENT OF WORK Overnight Shipping of packages for North Texas Dental Lab (Amarillo) Scope of Work: Contractor shall provide local pickup of dental cases and delivery to Amarillo VA Dental for repair of partials and dentures as well as fabrication of dentures, partials, nightguards, and fluoride trays. The contractor shall provide laboratory support including removable prosthodontic laboratory work for VAMC Amarillo. The contractor shall provide all materials and supplies necessary to complete each item. Acceptable supplies for removable prosthodontics include: Vitallium Alloy, SR- Ivocap High Impact Injectable Resin utilizing SR-Ivocap Injection Molding System and Trubyte IPN or equivalent denture teeth. Specific Requirements: The contractor shall use procedures, techniques and current technology that are considered to be the standards in the industry. The contractor shall provide items that meet the esthetic and functional requirements of the patient. The contractor shall be familiar with mould guides by Trubyte Bioblend and with shade guides by Trubyte Bioform and the VITA 3D Toothguide. High quality materials shall be utilized from ISO 9001/CE registered companies. Laboratory shall perform internal technical audits to ensure consistent, high quality, and timely restorations. The contractor shall provide a denture storage box for each completed denture case at no additional cost to VAMC Amarillo. The contractor shall provide the following dental laboratory services to the VA Medical Center, Amarillo, TX: Complete Dentures (including custom trays, bite rims, articulation, set-up, process and finish) Cast Partial Dentures (including custom trays, articulation, frameworks, bite rims, set-up, process and finish) Acrylic Partial Dentures with wrought wire clasps (including articulation) Hybrid Cast/Flexible Partial Dentures (including custom trays, articulation, frameworks, bite rims, set-up, process and finish) Complete and Partial Denture repair (repair of cast partial dentures will be completed using laser welding technique) Complete and Partial Denture relines and rebases Custom Occlusal Guards (including articulation) Valplast Flexible Partial Dentures Surgical stents ID dentures and partials Implant bar overdenture Precision cast retentive bars Precision and semi-precision attachments Fluoride application trays All Complete Dentures, Cast and Acrylic Partial Dentures, Flexible Partial Dentures and Custom Occlusal Guards shall be appropriately mounted using a Semi-Adjustable Articulator. The following materials shall be used in the fabrication of dental prostheses: Complete Dentures: Custom Trays fabricated using Light Tray Custom Tray Material Record Bases fabricated using Light Tray Custom Tray Material Occlusal Rims fabricated using Dentsply Occlusal Sticks Denture Teeth Setup using Dentsply Truwax Extra Hard Base Plate Wax Denture Teeth Wax Up using Modern Materials Red Base Plate Wax SR-Ivocap High Impact Injectable Resin utilizing the SR-Ivocap Injection Molding System Trubyte IPN or equivalent denture teeth. Cast Partial Dentures: Framework Partial Denture Master Casts fabricated using Die Stone Frameworks fabricated using Vitallium Alloy Custom Trays fabricated using Light Tray Custom Tray Material Record Bases fabricated using Light Tray Custom Tray Material Occlusal Rims fabricated using Dentsply Occlusal Sticks Denture Teeth Setup using Dentsply Truwax Extra Hard Base Plate Wax Denture Teeth Wax Up using Modern Materials Red Base Plate Wax SR-Ivocap High Impact Injectable Resin utilizing the SR-Ivocap Injection Molding System Trubyte IPN or equivalent denture teeth. Flexible Partial Dentures: Partial fabricated using Valplast Custom Trays fabricated using Light Tray Custom Tray Material Record Bases fabricated using Light Tray Custom Tray Material Occlusal Rims fabricated using Dentsply Occlusal Sticks Denture Teeth Setup using Dentsply Truwax Extra Hard Base Plate Wax Denture Teeth Wax Up using Modern Materials Red Base Plate Wax Trubyte IPN or equivalent denture teeth. Acrylic Partial Dentures: SR-Ivocap High Impact Injectable Resin utilizing the SR-Ivocap Injection Molding System Custom Trays fabricated using Light Tray Custom Tray Material Record Bases fabricated using Light Tray Custom Tray Material Occlusal Rims fabricated using Dentsply Occlusal Sticks Denture Teeth Setup using Dentsply Truwax Extra Hard Base Plate Wax Denture Teeth Wax Up using Modern Materials Red Base Plate Wax Acrylic Partial Denture Clasps will be of size 0.032 Retainer Wire Trubyte IPN or equivalent denture teeth. Relines and Rebases: Ivoclar Probase Cold Cure Resin Occlusal Guards: Hand waxed and heat cured using Impak material (Elastic Acrylic Resin) Surgical Stents: Clear SR-Ivocap High Impact Resin utilizing the SR-Ivocap Injection Molding System Tooth Material: Trubyte TM IPN or equivalent Occlusal Scheme: Monoplane, linear Anatomic, full balanced Lingualized, linear 33 */monoplane 30 *rational Functional/rational The contractor must identify the specific type of metal used in each prosthesis at the time of delivery. The contractor shall provide a thorough infection control process for all incoming and outgoing cases. All completed cases must be disinfected before the product is returned to VAMC Amarillo. Contractor shall provide local repair and reline service with 24-hour turnaround of all denture repairs and relines/rebases. The contractor shall provide, at no additional cost to the VA, professional case consultation with responsive and knowledgeable customer service. The contractor shall provide personalized shipping, shipment tracking and/or pickup and delivery services to ensure that casework arrives on time and safely at no additional cost to VAMC Amarillo. The contractor shall provide all packing and shipping materials at no additional cost to VAMC Amarillo. The attached Quality Assessment of Dental Laboratory Procedures will be used to measure/assess the quality of the product produced. Period of Performance: Period of performance shall be 1 base year with 4 one-year options. Dental cases shall be picked up between the hours of 8:00 a.m. to 4:00 p.m. Base Year 7-1-22 to 6-30-23 First Option Year 7-1-23 to 6-30-24 Second Option Year 7-1-24 to 6-30-25 Third Option Year 7-1-25 to 6-30-26 Fourth Option Year 7-1-26 to 6-30-27 Place of Performance: At Building 5809 S Western, Ste 230 North Texas Dental Laboratory, Amarillo, Texas 79110 (Not to be confused with North Texas Dental Laboratory Dallas Texas) The Contractor shall deliver and pick up at the following facilities: North Texas Dental Lab 5809 S Western, Ste 230 North Texas Dental Lab Driver Check in Requirements: North Texas Dental Lab will pick up dental cases from Amarillo VA Hospital at Dental VA Clinic and deliver packages to Amarillo VA Dental Clinic. Contracting Officer Technical Representatives: Prior to contract award, the Contracting Officer shall designate a VA Medical Center employee as the COTR. All work coordination shall be made through the COTR. The Contractor shall be provided a copy of the letter of delegation authorizing the COTR at the commencement of the term of this agreement. No other person shall be authorized to act in such capacity unless appointed in writing by the Contracting Officer. Information technology security requirements: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The contractor shall comply with all Federal laws and regulations the VA has developed when VA sensitive information is accessed, used, stored, generated, transmitted, or exchanged by and between VA and a contractor. The information made available to the contractor by VA for the performance of this contract will be used only for the purposes of performance under this contract. The C&A requirements do not apply to this requirement and a security accreditation package is not required. Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services in the application of complex adaptive system theory to health care organizations. Privacy Officer: Service Provider will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA. Safety Requirements: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default. Invoicing: Payment to be made weekly in arrears by certified invoices and must contain the contract number in addition to the requirements detailed in 52.212-4 (G) to be considered valid. All invoices shall be submitted to the VA Financial Service Center and emailed to the COTR. All invoices will reference the purchase order number assigned to the contract. Billing address: VA FSC PO Box 149971 Austin, TX 78714 Records Management: All records (administrative and program specific) created during the period of the contract belong to VA North Texas Health Care System(VANTXHCS) and must be returned to VANTXHCS at the end of the contract or destroyed in accordance to the VHA Record Control Schedule (RCS)10-1. SECURITY REQUIREMENTS: In reference to VHA Handbook 6500.6 Appendix A, Blocks 6 and 7, the equipment included in this contract does not capture and process ePHI over the hospital network. In reference to VHA Handbook 6500.6 Appendix A, Block 6, the equipment (or service) included in this contract does not involve connection of IT devices to a VA network. Therefore, C&A, SAP, & the following from App C do not apply. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. During install, training, warranty repair and/or maintenance provided by the contractor, they may have physical administrative access to the equipment included in this contract that capture and process ePHI. If warranty repair is done off-site, all hard drives or other data storage devices will be removed or purged of data, prior to release to contractor. Privacy and HIPAA Focused Training Web-Based (WBT): VA Course No. VA10203 is the official privacy training for VA employees, contractors, and volunteers who have access to protected health information (PHI) in the performance of their official duties or have access to VA systems (e.g. CPRS or VistA Web). All contracted employees will complete this training prior to being granted access to the subject equipment, or to the data therein. The training is available on the VA TMS website at: https:// www.tms.va.gov. Contractor will provide evidence of training to the COR, if requested. The Contractor personnel must follow all VA policies, standard operating procedures, applicable laws and regulations while on VA property. Violations of VA regulations and policies may result in citation and disciplinary measures for persons violating the law. The Contractor must obtain all necessary licenses and/or permits required to perform the work, except for software licenses that need to be procured from a Contractor or vendor in accordance with the requirements document.
