- Statement of Work Department of Veterans Affairs VA Ct. SPS Door Replacement A. The purpose of this request is to establish a contract to remove and replace the SPS Door Room 192 B. Location of VA facility West Haven VAMC 950 Campbell Ave West Haven, CT 06516-2770 A.) General Information 1. All contracted work shall be performed by a qualified tradesman according to the work being performed. 2. Installation shall include the following: Replace two doors, one opening with new vertical stiffened metal doors with glass window Install new strikes and latches Install fire rated vertical rod exit device (No bottom rods) One Door to have exit device trim (lever on pull side of door) Install trim protector bars Vertical Panic device, Rim Ext device. Services mounted Electrical connections are to be made by others Exit devices have electric latch retraction that will need to be tied into the existing operator. The contractor is responsible for removing and replacing the mechanical and hardware components. Requested Product description: 1. Protection Plate KIOSO 16"" x 32"" US3 The contractor shall preform the door replacement during normal business hours 7:30AM EST to 4: OOPM EST, Monday through Friday at a time and date agreeable to the VA and the contractor. When the install cannot be completed during normal business hours the contractor shall complete install on nights or weekends at no additional cost to the Government at a time and date agreeable to the VA and contractor. A. Debris Removal Upon completion of the job and as required daily, all materials and debris shall be removed from the station. Debris shall be disposed of by the contractor in accordance with applicable State Environmental Protection Regulations. B. Interference to Normal Functions 1) The contractor may be required to interrupt their work at any time as not to interfere with the function of the facility, including facility repair etc. 2) In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the Government. C. Additional Information 1) The Department of Veterans Affairs is a smoke free environment and smoking is not permitted at the facility. 2) COVID 19 precautions: Face coverings are required while on station. D. Due to 2020 COVID-19 safety. The facilities have COVID-19 mandates in place to support the facilities policy's regarding contracting work performed at the facility. Prior to commencement of work the contractor must check in with the COR/Supervisor of the Facilities Management Services/Bldg. 22. At the point of arrival, the contractor and all employees will be checked in through Bldg. 15 (FMS Engineering Office). In addition, there will be another COVID-19 prescreening prior to entering the Main Building. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents ,viii be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schcdu ks. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in suppo11 of this contract requiring the disclosure of infomiation, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and pi-oprictmy information.
