- The purpose of this Sources Sought Notice is issued as a means of conducting market research and does not constitute a request for proposal, an announcement of a forthcoming solicitation or a commitment by the Government in any way. The United States Department of Defense (DoD) on behalf of the Department of Health and Human Services (HHS) and Assistant Secretary for Preparedness and Response (ASPR) is seeking information to assess market availability and sourcing to purchase between 25,000,000 to 70,000,000 CBFCs �with immediate deliveries starting at contract award and all shipments no later than 120 days from contract award. The product shall be a multi-use, disposable face covering intended for a school-aged child between the age of 5 to 14 years old and offerors will have discretion to propose a product with face length and face width anticipated to fit a school-aged child between this age range. For any forthcoming Request for Proposal (RFP), offerors will be referred to Section 5.3 of ASTM 3502-21 for additional requirements (i.e., ear loops, adjustable straps, etc.). The Government intends to solicit and award to up to three (3) small businesses to satisfy this requirement; however, the Government reserves the right to make an award to one small business if necessary to satisfy requirement. �The contract(s) are anticipated to be a Firm Fixed Price, Definite Quantity, Total Small Business Set-Aside (SBSA) contracts. �� � The Government requires that only non-proprietary information be submitted in response to this Sources Sought. � Responses are required by no later than 17 May 2022. 2.��� Responses must include the following information: �������� a. �Company Name �������� b.� Address �������� c. CAGE/SAM Code �������� d. Point of Contact �������� e. Phone & Email �������� f.� Small � �������� g. Web page URL, if available. �������� h. Small business type or category you represent, if applicable. �������� i.� Identify as a U.S. or Foreign Owned Entity �������� j.� Indicate whether you are a manufacturer or distributor. If you are a distributor, identify the manufacturer that is producing the product you are providing. ������� k. Indicate current status of being registered in the System for Award Management (SAM) and Wide Area Workflow (WAWF). ������� 3. �Does your company self-certify that its product meets all the Model A requirements in ASTM Guide F3050?� If not, would your company self-certify its product meets all the Model A requirements in ASTM Guide F3050 prior to an award?� 4. Does your company have manufacturing located within the United States to meet any requirements or would manufacturing outside the US be required? If outside the US, does your company currently export to the United States? Please provide the country of origin(s). Does your company ship directly or through a distributor? 5. The Berry Amendment requires that certain textiles be produced in the United States if those fabrics are incorporated into the end products purchased by the Department of Defense.� Do your CBFC comply with this restriction?� Specifically, Does your CBFC include cotton and other natural fiber products, woven silk or woven silk blends, spun silk yarn for cartridge cloth, synthetic fabric or coated synthetic fabric (including all textile fibers and yarns that are for use in such fabrics), canvas products, or wool (whether in the form of fiber or yarn or contained in fabrics, materials, or manufactured articles), or elastic; if those fabrics are incorporated into your CBFC, are those fabrics produced in the United States?� If they are not produced in the United States, where are they produced? 6. Does your company offer individually packed masks and/or packed in three packs? If not 3-pack, is your company able to accommodate this request? 7.What is the maximum quantity your company can provide within 120 days of contract award, which is anticipated end of November 2022. What is the maximum quantity your company can provide on a monthly basis? 8. For the purpose of product testing in accordance with ASTM-F3502-21, would your company agree to provide at minimum 10 product demonstration models with specification sheet(s) to the Government free of charge at the time of proposal submittal?
